【摘要】：目的建立一套科學(xué)、全面、可行的縣級(jí)婦幼保健機(jī)構(gòu)發(fā)展能力評(píng)價(jià)指標(biāo)體系,用于我國(guó)縣級(jí)婦幼保健機(jī)構(gòu)發(fā)展能力綜合評(píng)價(jià)。方法本研究首先通過(guò)文獻(xiàn)研究、專家論證,確定發(fā)展能力評(píng)價(jià)框架,并結(jié)合全國(guó)婦幼保健機(jī)構(gòu)資源與運(yùn)營(yíng)情況監(jiān)測(cè)(簡(jiǎn)稱機(jī)構(gòu)監(jiān)測(cè))指標(biāo)設(shè)置備選指標(biāo)。利用2012年度機(jī)構(gòu)監(jiān)測(cè)數(shù)據(jù),采用相關(guān)系數(shù)法、因子分析法、聚類分析法及專家研討會(huì)法,以主客觀結(jié)合的方法進(jìn)行指標(biāo)篩選。采用德?tīng)柗茖＜以u(píng)分法確定指標(biāo)權(quán)重。利用2012年度機(jī)構(gòu)監(jiān)測(cè)指標(biāo)數(shù)據(jù),采用TOPSIS法、RSR法及組合評(píng)價(jià)對(duì)湖南省100所縣級(jí)婦幼保健機(jī)構(gòu)發(fā)展能力進(jìn)行綜合評(píng)價(jià),并隨機(jī)抽取其中20所機(jī)構(gòu),由湖南省省級(jí)婦幼衛(wèi)生行政部門(mén)領(lǐng)導(dǎo)和省級(jí)婦幼保健院管理專家對(duì)其發(fā)展能力進(jìn)行主觀評(píng)價(jià),比較兩種評(píng)價(jià)結(jié)果的一致性,以及與現(xiàn)實(shí)情況的符合程度,對(duì)評(píng)價(jià)指標(biāo)體系進(jìn)行驗(yàn)證。結(jié)果1.形成縣級(jí)婦幼保健機(jī)構(gòu)發(fā)展能力評(píng)價(jià)指標(biāo)體系。評(píng)價(jià)指標(biāo)體系包含3個(gè)一級(jí)指標(biāo)、8個(gè)二級(jí)指標(biāo)33個(gè)三級(jí)指標(biāo)。一級(jí)指標(biāo)包括資源配置、服務(wù)能力和發(fā)展?jié)摿?其中資源配置分為人力、物資、信息3個(gè)二級(jí)指標(biāo);服務(wù)能力分為轄區(qū)管理服務(wù)和院內(nèi)服務(wù)2個(gè)二級(jí)指標(biāo);發(fā)展?jié)摿Ψ譃闄C(jī)構(gòu)管理、經(jīng)濟(jì)運(yùn)營(yíng)、科研與學(xué)習(xí)3個(gè)二級(jí)指標(biāo)。2.確定了各級(jí)指標(biāo)的權(quán)重值。3個(gè)一級(jí)指標(biāo)的權(quán)重分別為0.33、0.40、0.27;8個(gè)二級(jí)指標(biāo)權(quán)重分別為 0.42、0.31、0.27、0.48、0.52、0.39、0.32、0.28。3.指標(biāo)體系驗(yàn)證結(jié)果。TOPSIS法與RSR法組合評(píng)價(jià)與主觀評(píng)價(jià)結(jié)果的一致性檢驗(yàn)kappa系數(shù)為0.211,同時(shí)評(píng)價(jià)結(jié)果與實(shí)際情況也比較相符。結(jié)論本研究基于機(jī)構(gòu)監(jiān)測(cè)系統(tǒng),采用了文獻(xiàn)研究、專家咨詢和專家研討會(huì)、數(shù)理統(tǒng)計(jì)分析等主客觀結(jié)合的方法,經(jīng)過(guò)確立評(píng)價(jià)框架和備選指標(biāo)、指標(biāo)篩選、指標(biāo)賦權(quán)、評(píng)價(jià)指標(biāo)體系驗(yàn)證等研究步驟,構(gòu)建了縣級(jí)婦幼保健機(jī)構(gòu)發(fā)展能力評(píng)價(jià)指標(biāo)體系。
[Abstract]:Objective to establish a scientific, comprehensive and feasible index system for evaluating the development ability of MCH institutions at county level in China. Experts demonstrated that an evaluation framework for capacity development was established and alternative indicators were established in conjunction with indicators for monitoring the resources and operations of national maternal and child health institutions (agency monitoring), using institutional monitoring data for 2012. Correlation coefficient method, factor analysis method, cluster analysis method and expert seminar method were used. The index is screened by combining subjective and objective methods. Delphi expert scoring method is used to determine the index weight. The TOPSIS method is used to determine the index data of institutional monitoring in 2012. The development ability of 100 county maternal and child health care institutions in Hunan Province was evaluated by RSR method and combined evaluation, and 20 of them were randomly selected. The development ability of the provincial MCH administrative department of Hunan Province and the management experts of provincial MCH hospital were evaluated subjectively to compare the consistency of the two evaluation results and the degree of consistency with the actual situation. The evaluation index system was validated. Results 1. The evaluation index system of the development ability of MCH institutions at county level was formed. The evaluation index system included three first-level indicators. 2. The first level includes resource allocation, service capacity and development potential; The allocation of resources is divided into three levels: manpower, material and information. The service ability is divided into two levels: management service and hospital service. The development potential is divided into three secondary indexes, I. e., institutional management, economic operation, scientific research and learning. The weights of the eight secondary indexes were 0.42 ~ 0.31 ~ 0.27 ~ 0.28 ~ 0.52 ~ 0.39 ~ 0.32 respectively. 0.28.3.The kappa coefficient of the combination evaluation and subjective evaluation of TOPSIS method and RSR method is 0.211. At the same time, the evaluation results are also consistent with the actual situation. Conclusion this study is based on the institutional monitoring system, using literature research, expert consultation and expert seminars, mathematical statistical analysis and other subjective and objective methods. The evaluation index system for the development ability of MCH institutions at county level was constructed by establishing the evaluation framework and alternative indicators, selecting indicators, empowering indicators, and verifying the evaluation index system.
