【摘要】：蚊蟲對(duì)人類危害極大,不僅吸血騷擾,而且傳播多種蚊媒傳染病。我國(guó)流行的蚊媒疾病有瘧疾、流行性乙型腦炎、登革熱等,法定傳染病中的1/3以上與病媒生物有關(guān)。全國(guó)愛(ài)衛(wèi)會(huì)于1997年開(kāi)展創(chuàng)建國(guó)家衛(wèi)生鎮(zhèn)(縣城)活動(dòng),地方各級(jí)政府積極響應(yīng)、廣大人民群眾積極參與。通過(guò)創(chuàng)建國(guó)家衛(wèi)生城市(區(qū)、鎮(zhèn))活動(dòng),城鄉(xiāng)環(huán)境衛(wèi)生明顯改善,除四害和病媒生物防制效果顯著,群眾健康水平有所提升。病媒生物防制是愛(ài)國(guó)衛(wèi)生工作的重要內(nèi)容之一。2010年全國(guó)愛(ài)衛(wèi)辦下發(fā)的《國(guó)家衛(wèi)生鄉(xiāng)鎮(zhèn)(縣城)標(biāo)準(zhǔn)》明確提出要通過(guò)綜合防制,有效控制鼠、蚊、蠅、蟑螂等病媒生物。按照國(guó)家規(guī)定的標(biāo)準(zhǔn),蚊、蠅、蟑螂密度至少有一項(xiàng)達(dá)到,其他項(xiàng)不超過(guò)標(biāo)準(zhǔn)的3倍。2005年我國(guó)建立了覆蓋19個(gè)省43個(gè)市的病媒生物監(jiān)測(cè)網(wǎng)絡(luò),已形成從監(jiān)測(cè)、預(yù)警、防制到管理的完整體系。2006年成立衛(wèi)生部病媒生物控制標(biāo)準(zhǔn)專業(yè)委員會(huì)后,全國(guó)愛(ài)衛(wèi)辦指導(dǎo)標(biāo)委會(huì)出臺(tái)了44項(xiàng)相關(guān)標(biāo)準(zhǔn)。2009年衛(wèi)生部和全國(guó)愛(ài)衛(wèi)辦發(fā)布《關(guān)于印發(fā)〈病媒生物預(yù)防控制管理規(guī)定〉的通知》,這是我國(guó)第一部病媒生物防制相關(guān)規(guī)定,科學(xué)規(guī)范了病媒生物預(yù)防控制管理工作。2015年國(guó)務(wù)院下發(fā)《關(guān)于進(jìn)一步加強(qiáng)新時(shí)期愛(ài)國(guó)衛(wèi)生工作的意見(jiàn)》中明確指出,要科學(xué)預(yù)防控制病媒生物,實(shí)施以環(huán)境治理為主的綜合預(yù)防控制策略,有針對(duì)性地組織開(kāi)展除“四害”活動(dòng)。本研究是基于2013年國(guó)家衛(wèi)生和計(jì)劃生育委員會(huì)疾病預(yù)防控制局組織開(kāi)展的全國(guó)病媒生物防制基本信息調(diào)查,調(diào)查涉及全國(guó)2334個(gè)城市,有效問(wèn)卷為2290個(gè),其中縣城1439個(gè),國(guó)家衛(wèi)生縣163個(gè),非國(guó)家衛(wèi)生縣1199個(gè),滅蚊達(dá)標(biāo)縣403個(gè),滅蚊未達(dá)標(biāo)縣652個(gè)。調(diào)查主要內(nèi)容包括基本信息、病媒生物防制工作情況和病媒生物防制專業(yè)機(jī)構(gòu)情況等。本研究旨在通過(guò)德?tīng)柗品êY選出影響滅蚊工作的主要因素,對(duì)調(diào)查的國(guó)家衛(wèi)生縣和非國(guó)家衛(wèi)生縣、滅蚊達(dá)標(biāo)縣和滅蚊未達(dá)標(biāo)縣分別進(jìn)行對(duì)照,對(duì)國(guó)家衛(wèi)生縣和非國(guó)家衛(wèi)生縣的各項(xiàng)影響因素情況進(jìn)行單因素分析,對(duì)滅蚊達(dá)標(biāo)縣和滅蚊未達(dá)標(biāo)縣的各項(xiàng)影響因素情況進(jìn)行單因素和多因素綜合分析,綜合對(duì)照分析創(chuàng)建國(guó)家衛(wèi)生縣城是否對(duì)蚊媒防制工作有顯著促進(jìn)作用。以此為理論基礎(chǔ),提出下一步政策建議,為各地以創(chuàng)建國(guó)家衛(wèi)生縣城為抓手,推動(dòng)蚊媒防制工作科學(xué)合理開(kāi)展提供科學(xué)理論依據(jù)。本研究主要結(jié)果為:創(chuàng)建國(guó)家衛(wèi)生縣對(duì)于蚊媒防制工作有顯著促進(jìn)作用。滅蚊達(dá)標(biāo)是蚊媒防制工作的最重要效果評(píng)價(jià)指標(biāo),滅蚊達(dá)標(biāo)縣和滅蚊未達(dá)標(biāo)縣的多因素分析結(jié)果顯示,是否是國(guó)家衛(wèi)生縣與滅蚊達(dá)標(biāo)有明顯關(guān)聯(lián)。國(guó)家衛(wèi)生縣滅蚊達(dá)標(biāo)比例是非國(guó)家衛(wèi)生縣滅蚊達(dá)標(biāo)比例的一倍,國(guó)家衛(wèi)生縣在政府重視、政策保障、經(jīng)費(fèi)投入、防制機(jī)構(gòu)建設(shè)、專業(yè)隊(duì)伍建設(shè)、社會(huì)化服務(wù)機(jī)構(gòu)建設(shè)管理、密度監(jiān)測(cè)和抗藥性監(jiān)測(cè)工作等情況均優(yōu)于非衛(wèi)生縣。說(shuō)明創(chuàng)建國(guó)家衛(wèi)生縣對(duì)蚊媒防制工作具有顯著促進(jìn)作用。
[Abstract]:Mosquitoes do harm to human beings, not only vampire harassment, and the spread of a variety of mosquito borne infectious disease epidemic in China. Mosquito borne diseases include malaria, Japanese encephalitis, dengue fever and other infectious diseases, more than in the 1/3 and vector. The National Patriotic Health Campaign Committee in 1997 to carry out the creation of national health town (county) activities, the local government actively response, active participation of the masses. Through the creation of national health city (District, town), the urban and rural environmental sanitation improvement, four pests and pest control effect, the health level of people has improved. Based on this theory, put forward policy recommendations for all to create a national health county as the starting point, to promote the prevention and control of mosquitoes carry out scientific and reasonable to provide a theoretical basis for the main results. This study is: to create a national health county has a significant role for mosquito prevention mosquito. The standard is the most important index effect of mosquito prevention, mosquito and mosquito did not meet the standards of county county, the multivariate analysis results show that whether the national health county is obviously associated with the mosquito mosquito standard of national health county standards. The proportion is a non national health standard times County mosquito proportion, the national health county in the attention of the government policy, funding, and institutional construction, the construction of the professional team, The construction and management of social service institutions, density monitoring and resistance monitoring are all better than those of non health counties. This indicates that the establishment of national health county has a significant role in promoting mosquito control.
