【摘要】：目的:探討EBV編碼的潛伏膜蛋白2A(LMP2A)與上皮-間質(zhì)轉(zhuǎn)化(EMT)標(biāo)志物E-鈣粘素(E-Cadherin)和纖維連接蛋白在鼻咽癌(NPC)中的表達(dá)及與臨床病理學(xué)參數(shù)的關(guān)系。方法:采用免疫組織化學(xué)SP法檢測LMP2A、E-Cadherin和fibronectin蛋白在32例鼻咽黏膜慢性炎、56例NPC和18例NPC淋巴結(jié)轉(zhuǎn)移灶中的表達(dá)。結(jié)果:1LMP2A在NPC及其淋巴結(jié)轉(zhuǎn)移灶中的陽性表達(dá)率分別為89.3%(50/56)和77.8%(14/18),均明顯高于鼻咽黏膜慢性炎(37.5%,12/32)(均P0.01);E-Cadherin在NPC及其淋巴結(jié)轉(zhuǎn)移灶中的正常表達(dá)率分別為33.9%(19/56)和5.6%(1/18),均明顯低于鼻咽黏膜慢性炎(90.6%,29/32)(均P0.01);fibronectin在NPC及其淋巴結(jié)轉(zhuǎn)移灶中的陽性表達(dá)率分別為83.9%(47/56)和72.2%(13/18),均明顯高于鼻咽黏膜慢性炎(28.1%,9/32)(均P0.01)。2NPC中LMP2A與E-Cadherin正常表達(dá)呈負(fù)相關(guān)(r=-0.387,P0.01)、與fibronectin表達(dá)呈正相關(guān)(r=0.421,P0.01)。3LMP2A、E-Cadherin和fibronectin表達(dá)與NPC患者N分期和臨床分期密切相關(guān)(均P0.05),與M分期無相關(guān)性(P0.05);LMP2A、E-Cadherin表達(dá)與T分期也密切相關(guān)(均P0.01)。結(jié)論:LMP2A、fibronectin在NPC中表達(dá)升高,而E-Cadherin正常表達(dá)則降低;LMP2A可能通過下調(diào)E-Cadherin和上調(diào)fibronectin表達(dá)來介導(dǎo)EMT促進(jìn)NPC淋巴結(jié)轉(zhuǎn)移和惡性進(jìn)展。

【正文快照】： 鼻咽癌(nasopharyngeal carcinoma,NPC)是一種EB病毒(epstein-barr virus,EBV)相關(guān)性癌,以我國南方地區(qū)尤其是廣東省多見,臨床上約60%的NPC患者被發(fā)現(xiàn)時已到中晚期,晚期患者放療后30%~40%有遠(yuǎn)距離轉(zhuǎn)移和復(fù)發(fā),遠(yuǎn)距離轉(zhuǎn)移后,預(yù)后差,中位生存時間12個月〔1〕。放化療抵抗和侵襲
