【摘要】：目的:探討EBV編碼的潛伏膜蛋白2A(LMP2A)與上皮-間質轉化(EMT)標志物E-鈣粘素(E-Cadherin)和纖維連接蛋白在鼻咽癌(NPC)中的表達及與臨床病理學參數的關系。方法:采用免疫組織化學SP法檢測LMP2A、E-Cadherin和fibronectin蛋白在32例鼻咽黏膜慢性炎、56例NPC和18例NPC淋巴結轉移灶中的表達。結果:1LMP2A在NPC及其淋巴結轉移灶中的陽性表達率分別為89.3%(50/56)和77.8%(14/18),均明顯高于鼻咽黏膜慢性炎(37.5%,12/32)(均P0.01);E-Cadherin在NPC及其淋巴結轉移灶中的正常表達率分別為33.9%(19/56)和5.6%(1/18),均明顯低于鼻咽黏膜慢性炎(90.6%,29/32)(均P0.01);fibronectin在NPC及其淋巴結轉移灶中的陽性表達率分別為83.9%(47/56)和72.2%(13/18),均明顯高于鼻咽黏膜慢性炎(28.1%,9/32)(均P0.01)。2NPC中LMP2A與E-Cadherin正常表達呈負相關(r=-0.387,P0.01)、與fibronectin表達呈正相關(r=0.421,P0.01)。3LMP2A、E-Cadherin和fibronectin表達與NPC患者N分期和臨床分期密切相關(均P0.05),與M分期無相關性(P0.05);LMP2A、E-Cadherin表達與T分期也密切相關(均P0.01)。結論:LMP2A、fibronectin在NPC中表達升高,而E-Cadherin正常表達則降低;LMP2A可能通過下調E-Cadherin和上調fibronectin表達來介導EMT促進NPC淋巴結轉移和惡性進展。
