【摘要】： 目的 研究非甾體類抗炎藥在不同的給藥時(shí)機(jī)對(duì)白內(nèi)障術(shù)后黃斑中心凹厚度的影響,以及糖尿病患者與單純性白內(nèi)障患者在給藥時(shí)機(jī)上是否存在差異。 方法 將于我院施行白內(nèi)障超聲乳化吸除聯(lián)合后房型人工晶體植入手術(shù)的患者90例(90只眼)納入試驗(yàn),其中糖尿病患者46例(46只眼),單純性白內(nèi)障患者44例(44只眼),所有患者隨機(jī)分入試驗(yàn)組與對(duì)照組,每組各有23例糖尿病患者和22例單純性白內(nèi)障患者,試驗(yàn)組：術(shù)前1天開始使用普拉洛芬滴眼液,術(shù)后第1天繼續(xù)使用普拉洛芬滴眼液至術(shù)后30天；對(duì)照組：術(shù)后第1天開始使用普拉洛芬滴眼液至術(shù)后30天,兩組患者均于術(shù)后第1天開始使用地塞米松／托布霉素滴眼液,所有滴眼液均日4次點(diǎn)眼。分別在術(shù)前7天內(nèi),術(shù)后第7天和術(shù)后第30天采用光學(xué)相干斷層掃描儀定量測(cè)定黃斑中心凹厚度。 結(jié)果 試驗(yàn)組與對(duì)照組的黃斑中心凹厚度在術(shù)后第7天無顯著性差異(P0.05),在術(shù)后第30天有顯著性差異(P0.05)。試驗(yàn)組與對(duì)照組中糖尿病患者的黃斑中心凹厚度及黃斑中心凹厚度增加值在術(shù)后第7天及第30天均有顯著性差異(P0.05)；試驗(yàn)組與對(duì)照組中單純性白內(nèi)障患者的黃斑中心凹厚度和黃斑中心凹厚度增加值在術(shù)后第7天及術(shù)后第30天均無顯著性差異(P0.05)。 結(jié)論 本研究的實(shí)驗(yàn)數(shù)據(jù)顯示單純性白內(nèi)障患者,術(shù)前應(yīng)用非甾體類抗炎藥與術(shù)后應(yīng)用在術(shù)后黃斑中心凹厚度上無顯著性差異；然而,糖尿病患者在白內(nèi)障術(shù)前應(yīng)用非甾體抗炎藥滴眼液比術(shù)后應(yīng)用可顯著降低術(shù)后黃斑中心凹視網(wǎng)膜增厚的程度。本研究結(jié)果提示,當(dāng)治療糖尿病患者的白內(nèi)障時(shí),建議在術(shù)前開始應(yīng)用非甾體抗炎藥滴眼液,以減輕術(shù)后黃斑中心凹厚度增加,從而有助于降低糖尿病患者白內(nèi)障術(shù)后發(fā)生黃斑水腫的風(fēng)險(xiǎn)。

[Abstract]:Purpose To study the effect of non-opioid anti-inflammatory drugs on the thickness of the macular center after cataract extraction at different time of administration, as well as whether the patients with diabetes and simple cataracts were present at the time of administration. in difference Methods 90 patients (90 eyes) of post-chamber intraocular lens implantation in our hospital were included in the test, including 46 (46 eyes) and simple cataract in the patients with diabetes. There was a significant difference in the 30th day after the operation (P0.05). The thickness of the fovea and the thickness of the concave thickness of the macula in the diabetic patients in the test group and the control group were 7 days after operation. There was a significant difference between the first and the 30th day (P0.05). The thickness of the fovea and the thickness of the concave thickness of the macula in the patients with simple cataract in the test group and the control group were 7 days after operation. post There was no significant difference in 30 days (P0.05). There was no significant difference in the thickness of the post-operative macula. However, the non-diabetic anti-inflammatory drug was applied to the diabetic patients before and after the cataract surgery. The results of this study suggest that the non-diabetic anti-inflammatory drug eye drops should be applied before the operation in order to reduce the thickness of the macular hole after operation.
