【摘要】： 目的 1.研究CYR61 (cysteine-rich 61; CCN1)對脈絡(luò)膜視網(wǎng)膜內(nèi)皮細(xì)胞(RF／6A細(xì)胞系)增殖、遷移及形成管狀結(jié)構(gòu)的影響。 2.研究CYR61在糖尿病小鼠和正常小鼠視網(wǎng)膜的表達(dá)和分布情況及其差異。 3.研究增殖性糖尿病視網(wǎng)膜病變(PDR)患者玻璃體CYR61的表達(dá)水平,以及CYR61在視網(wǎng)膜增殖膜內(nèi)的表達(dá)和分布。 方法 1.通過MTT比色實驗觀察CYR61對RF／6A細(xì)胞增殖能力的影響：應(yīng)用Transwell小室觀察CYR61作用下,RF／6A細(xì)胞遷移能力的變化；采用Matrigel基質(zhì)膠,觀察CYR61對RF／6A細(xì)胞形成管狀結(jié)構(gòu)的影響。 2.腹腔注射鏈脲佐菌素(STZ)制作糖尿病小鼠模型,通過免疫組織化學(xué)方法觀察CYR61在糖尿病小鼠視網(wǎng)膜的表達(dá)和分布,利用逆轉(zhuǎn)錄-聚合酶鏈反應(yīng)(RT-PCR)方法分析視網(wǎng)膜Cyr61 mRNA的表達(dá)情況。 3.收集玻璃體切除手術(shù)患者的玻璃體液和視網(wǎng)膜前膜,利用酶聯(lián)免疫吸附實驗(ELISA)方法測定PDR患者和非糖尿病患者玻璃體CYR61的濃度,利用免疫組織化學(xué)方法觀察CYR61在PDR、增殖性玻璃體視網(wǎng)膜病變(PVR)和特發(fā)性黃斑前膜患者的視網(wǎng)膜前膜中的表達(dá)和分布情況。 結(jié)果 1.用不同濃度的CYR61(0,5ng/mL, 10ng/mL,50ng/mL, 100ng/mL,500ng/mL)干預(yù)RF／6A細(xì)胞72h,隨著CYR61濃度增加,MTT比色實驗的吸光度A490nm值升高,其差異有統(tǒng)計學(xué)意義(p=0.000)；400ng/mL Cry61干預(yù)RF／6A細(xì)胞不同時間(0,12h,24h,48h,72h),隨著時間延長,A490nm值升高,其差異有統(tǒng)計學(xué)意義(Jp=0.000)。Transwell小室實驗中,隨著CYR61濃度增加,遷移的RF／6A細(xì)胞增加(p=0.000),400ng/ml CYR61促進(jìn)細(xì)胞遷移的能力與80ng/ml VEGF相當(dāng),而加入抗CYR61抗體則可以起到顯著的抑制作用。Matrigel基質(zhì)膠實驗中,隨著CYR61濃度增加,RF／6A細(xì)胞形成的管狀結(jié)構(gòu)增多(p=0.000),400ng/ml CYR61與80ng/ml VEGF的作用相當(dāng),而加入抗CYR61抗體則可以起到顯著的抑制作用。 2.在正常小鼠的視網(wǎng)膜,CYR61在神經(jīng)節(jié)細(xì)胞層、內(nèi)叢狀層、外從狀層和光感受器層均有表達(dá),其中外從狀層表達(dá)較強(qiáng),其余各層弱陽性染色；在糖尿病小鼠的視網(wǎng)膜,CYR61同樣在以上各層表達(dá),但神經(jīng)節(jié)細(xì)胞層和內(nèi)叢狀層的表達(dá)較正常小鼠明顯增強(qiáng),神經(jīng)節(jié)細(xì)胞胞漿內(nèi)出現(xiàn)團(tuán)塊狀的陽性染色。糖尿病小鼠視網(wǎng)膜Cyr61 mRNA的表達(dá)較正常對照組增加,差異具有統(tǒng)計學(xué)意義(p=0.009)。 3.PDR組玻璃體CYR61濃度為97.53±34.13 (41.21～154.74) ng/mL,對照組玻璃體CYR61濃度為33.92±18.68 (6.24～70.04) ng/mL,兩組有顯著性差異(p=0.000)；視網(wǎng)膜和增殖膜新生血管豐富者玻璃體CYR61明顯高于無明顯新生血管者(p＝0.001)；未發(fā)現(xiàn)玻璃體CYR61與患者年齡、糖尿病病程及術(shù)前空腹血糖有關(guān)(p0.05)。CYR61分布于視網(wǎng)膜前膜的細(xì)胞胞漿中,在PDR視網(wǎng)膜前增殖膜中管腔樣結(jié)構(gòu)的管壁細(xì)胞高度表達(dá),在其它細(xì)胞也可見散在的陽性染色；PVR視網(wǎng)膜前增殖膜及特發(fā)性黃斑前膜中,有散在的陽性染色細(xì)胞。 結(jié)論 CYR61在體外可以促進(jìn)脈絡(luò)膜視網(wǎng)膜內(nèi)皮細(xì)胞增殖、遷移及形成管狀結(jié)構(gòu)；糖尿病小鼠視網(wǎng)膜Cyr61 mRNA和蛋白的表達(dá)均較正常小鼠升高：PDR患者玻璃體CYR61含量明顯高于非糖尿病患者,CYR61表達(dá)于PDR視網(wǎng)膜前增殖膜,尤其在管腔樣結(jié)構(gòu)的管壁細(xì)胞高度表達(dá)。CYR61很可能參與糖尿病視網(wǎng)膜病變的發(fā)病,在PDR新生血管的形成過程中起一定的作用。
[Abstract]:Purpose 1.
