鼻咽癌適形調(diào)強(qiáng)放療中涎腺功能的全面保護(hù)

【摘要】:目的:探討鼻咽癌適形調(diào)強(qiáng)放療(intensity modulate radiation therapy,IMRT)患者各涎腺功能的保護(hù)方法。方法:選取2010年3月至2012年11月101例連續(xù)于中山大學(xué)腫瘤醫(yī)院接受IMRT鼻咽癌患者,勾畫(huà)危及器官,包括腮腺、頜下腺、口腔,于3、6、12、18、24個(gè)月復(fù)查時(shí)采用面對(duì)面訪談式問(wèn)卷調(diào)查評(píng)分口干程度情況,并結(jié)合劑量體積直方圖(dose-volume histograms,DVH)進(jìn)行分析。結(jié)果:腮腺平均劑量(MD)為37.4 Gy(患側(cè)),33.8 Gy(健側(cè));頜下腺M(fèi)D為51.6 Gy(患側(cè)),45.7 Gy(健側(cè));口腔MD為38.2 Gy。77.2%(78/101)患者放療結(jié)束后6個(gè)月口干癥狀明顯改善,1年后中度(G3)以上口干患者5.0%(5/101)。結(jié)論:隨著時(shí)間的推移,放療后口干燥癥明顯改善;限定至少一側(cè)腮腺V30~35≤50.0%,至少一側(cè)頜下腺V40~45≤66.7%~50.0%,口腔MD40 Gy可有效保護(hù)唾液腺功能。
[Abstract]:Objective: to investigate the protective method of salivary gland function in patients with nasopharyngeal carcinoma (NPC) treated with conformal intensity modulation radiotherapy (intensity modulate radiation therapy,IMRT). Methods: from March 2010 to November 2012, 101 consecutive patients with IMRT nasopharyngeal carcinoma were enrolled in the Cancer Hospital of Sun Yat-sen University. In 12, 18, 24 months of review, face-to-face interview questionnaire was used to evaluate the dryness of the mouth, and the dose volume histogram (dose-volume histograms,DVH) was used to analyze the dry degree of the mouth. Results: the average dose of (MD) was 37.4 Gy (affected side) and 33.8 Gy (healthy side), and the MD of submandibular gland was 51.6 Gy (affected side) and 45.7 Gy (healthy side), respectively.
