【摘要】： 【目的】探討抗炎治療在蒸發(fā)過(guò)強(qiáng)型干眼患者中的作用,比較常見(jiàn)抗炎藥物典必殊(0.3%妥布霉素/地塞米松滴眼液)和辰景(雙氯芬酸鈉滴眼液)的抗炎差異。 【研究方法】將38例(76眼)診斷為蒸發(fā)過(guò)強(qiáng)型干眼患者,隨機(jī)分為三組(對(duì)照組20眼,糖皮質(zhì)激素治療組32眼,非甾體抗炎藥治療組24眼),對(duì)照組單純使用人工淚液+抗生素滴眼液+瞼板腺局部物理治療,激素治療組和非甾體抗炎藥物治療組則分別在對(duì)照組治療的基礎(chǔ)上分別加用皮質(zhì)類固醇滴眼液——典必殊(0.3%妥布霉素/地塞米松滴眼液)、非甾體抗炎藥物——辰景(雙氯芬酸鈉滴眼液)治療1個(gè)月,分別在治療前、治療后1周、1月進(jìn)行癥狀評(píng)分、視力、裂隙燈檢查(瞼緣評(píng)分及結(jié)膜充血程度觀察等)、淚膜破裂時(shí)間檢測(cè)、角膜熒光素染色檢查、淚液分泌試驗(yàn)檢查以及眼壓檢測(cè);并做對(duì)比。 【結(jié)果】①治療前采用完全隨機(jī)設(shè)計(jì)的單因素方差分析三組進(jìn)行兩兩比較,各組間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);②三組癥狀評(píng)分、淚膜破裂時(shí)間、角膜染色評(píng)分、淚液分泌實(shí)驗(yàn)、瞼緣評(píng)分治療前與治療后一周、一月分別行配對(duì)t檢驗(yàn)差異均有統(tǒng)計(jì)學(xué)意義(P0.05),提示三種治療均有效;③治療后一周:抗炎藥物組(激素組和非甾體抗炎藥物組)與對(duì)照組相比:淚膜破裂時(shí)間和瞼緣評(píng)分差異有顯著的統(tǒng)計(jì)學(xué)意義(P0.05),且以上各項(xiàng)指標(biāo)均值抗炎組優(yōu)于對(duì)照治療組;激素組與非甾體抗炎藥物組相比:癥狀評(píng)分、BUT和瞼緣評(píng)分差異有顯著的統(tǒng)計(jì)學(xué)意義(P0.05),且以上各項(xiàng)指標(biāo)均值激素組優(yōu)于非甾體藥物組;④治療后一個(gè)月:抗炎組(激素組和非甾體抗炎藥物組)與對(duì)照組相比:癥狀評(píng)分、BUT、淚液分泌實(shí)驗(yàn)和瞼緣評(píng)分差異有顯著的統(tǒng)計(jì)學(xué)意義(P0.05),且激素組以上各項(xiàng)指標(biāo)均值明顯優(yōu)于對(duì)照組;辰景組癥狀評(píng)分、BUT、瞼緣評(píng)分明顯優(yōu)于對(duì)照組,但角膜染色評(píng)分均值辰景組對(duì)照組,說(shuō)明辰景組治療后一月角膜情況較對(duì)照組療效差;激素組與非甾體藥物組相比:淚膜破裂時(shí)間和瞼緣評(píng)分差異有顯著的統(tǒng)計(jì)學(xué)意義(P0.05),且以上各項(xiàng)指標(biāo)均值激素組優(yōu)于非甾體藥物組; 【結(jié)論】三組患者的癥狀和體征均有不同程度的改善,抗炎藥物組效果優(yōu)于對(duì)照組,抗炎治療對(duì)蒸發(fā)過(guò)強(qiáng)型干眼有效;皮質(zhì)類固醇激素可以顯著的改善蒸發(fā)過(guò)強(qiáng)型干眼患者的癥狀和體征,抗炎效果優(yōu)于非甾體抗炎藥物。
[Abstract]:[objective] to investigate the role of anti-inflammatory therapy in evaporative dry eye patients, and to compare the anti-inflammatory effects of dipyridamole (0.3% tobramycin / dexamethasone eye drops) and diclofenac sodium eye drops (diclofenac sodium eye drops) on the anti-inflammatory effects of dipyridamole (0.3% tobramycin / dexamethasone eye drops). [methods] 38 patients (76 eyes) with hyperevaporative dry eyes were randomly divided into three groups: control group (20 eyes), glucocorticoid treatment group (32 eyes) and non-steroidal anti-inflammatory drug group (24 eyes). In the control group, only artificial lacrimal antibiotics were used for local physiotherapy of palpebral gland. Corticosteroid eye drops (0.3% tobramycin / dexamethasone eye drops) were added to the steroid treatment group and the non-steroidal anti-inflammatory drug treatment group respectively on the basis of the control group. The non-steroidal anti-inflammatory drug Chenjing (diclofenac sodium eye drops) was treated for 1 month. Symptom score, visual acuity, slit lamp examination (eyelid margin score and conjunctival congestion degree observation, etc.) were performed before, 1 week and 1 month after treatment, respectively. Lacrimal film rupture time, corneal fluorescein staining, lacrimal secretion test and intraocular pressure (IOP) were measured. And make a comparison. [results] (1) there was no significant difference among the three groups by single-factor ANOVA, which was completely randomly designed before treatment (P0.05); 2There were statistically significant differences in symptom score, tear film rupture time, corneal staining score, lacrimal secretion test, eyelid margin score before and after treatment one week and one month after treatment (P0.05). It is suggested that all three treatments are effective. 3 one week after treatment: compared with the control group, the lacrimal film rupture time and eyelid margin score in the anti-inflammatory drug group (hormone group and non-steroidal anti-inflammatory drug group) were significantly different (P0.05). The mean value of the above indexes in the anti-inflammatory group was better than that in the control group. Compared with non-steroidal anti-inflammatory drug group, hormone group had significant difference in symptom score, BUT and palpebral margin score (P0.05), and the above indexes were better in hormone group than in non-steroidal drug group. 4 one month after treatment: there was significant difference in symptom score, BUT, lacrimal secretion test and eyelid margin score between the anti-inflammatory group (hormone group and non-steroidal anti-inflammatory drug group) and the control group (P0.05). The average value of above indexes in hormone group was obviously better than that in control group. The symptom score and BUT, eyelid margin score in Chenjing group were significantly better than those in control group, but the corneal staining scores in Chenjing group were significantly higher than those in control group, indicating that the corneal condition in Chenjing group was worse than that in control group one month after treatment. Compared with the non-steroidal drug group, the lacrimal film rupture time and eyelid margin score were significantly different in hormone group (P0.05), and the above indexes were better in hormone group than in non-steroidal drug group. [conclusion] the symptoms and signs of the three groups were improved to some extent, the effect of anti-inflammatory drugs group was better than that of the control group, and the anti-inflammatory treatment was effective to dry eyes with excessive evaporation. Corticosteroids can significantly improve the symptoms and signs of patients with evaporative hyperintensity dry eyes, and the anti-inflammatory effect is better than that of non-steroidal anti-inflammatory drugs.
