【摘要】： 目的:觀察研究激光配合中藥(活血化淤,益氣養(yǎng)陰兼以利水)治療糖尿病視網(wǎng)膜病變的臨床療效。 方法:選2008年1月到2010年2月于吉林省中醫(yī)院(長(zhǎng)春中醫(yī)藥大學(xué)附屬醫(yī)院)眼科門(mén)診及療區(qū)住院的糖尿病性視網(wǎng)膜病變患者50例(78眼),其中29例患者雙眼符合入選標(biāo)準(zhǔn),21例患者單眼符合入選條件。將29例雙眼符合入選標(biāo)準(zhǔn)的患者,按隨機(jī)分配的方式分配到治療組15例(30眼),對(duì)照組14例(28眼);將21例(21眼)單眼符合入選標(biāo)準(zhǔn)的患者,按隨機(jī)分配的方式選分配到治療組11例(11眼),對(duì)照組10例(10眼)。治療組25例(40眼)應(yīng)用激光配合中藥治療,對(duì)照組25例(38眼)僅接受激光治療,觀察兩組的臨床有效率。入選患者均進(jìn)行全面的眼科檢查,治療后各隨訪15天、30天,觀察兩組患者治療前后視力、眼底改變及眼底血管造影結(jié)果的變化情況,分析兩組治療的效果,并做出相應(yīng)的結(jié)論。 結(jié)果:(1)治療一個(gè)月后治療組視網(wǎng)膜出血與滲出及微動(dòng)脈瘤吸收的有效率是92.5%,對(duì)照組的有效率是77.8%;兩組相比有統(tǒng)計(jì)學(xué)意義(P0.05);(2)治療1個(gè)月后治療組黃斑水腫消失的有效率是94.1%,對(duì)照組的有效率是71.4%;兩組相比有統(tǒng)計(jì)學(xué)意義(P0.05);(3)治療1個(gè)月后視力提高治療組總有效率是82.5%,對(duì)照組總有效率是52.8%;兩組組比較有統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論:治療組的療效優(yōu)于對(duì)照組,表現(xiàn)在治療組治療1個(gè)月時(shí)視網(wǎng)膜出血、滲出吸收的療效顯著優(yōu)于對(duì)照組,治療組治療1個(gè)月時(shí)黃斑水腫吸收、微動(dòng)脈瘤消失及視力提高的療效顯著優(yōu)于對(duì)照組,表明中藥配合激光治療糖尿病視網(wǎng)膜病變具有一定優(yōu)勢(shì)。
[Abstract]:Aim: to observe and study the clinical effect of laser combined with traditional Chinese medicine (activating blood circulation and removing blood stasis, tonifying qi and nourishing yin combined with diuresis) in treating diabetic retinopathy. Methods: from January 2008 to February 2010, 50 patients (78 eyes) with diabetic retinopathy who were hospitalized in the ophthalmology clinic and treatment area of Jilin traditional Chinese Medicine Hospital (affiliated Hospital of Changchun University of traditional Chinese Medicine) were selected. One eye of 21 patients met the selection criteria. Twenty-nine patients who met the criteria were randomly assigned to 15 cases (30 eyes) in the treatment group and 14 cases (28 eyes) in the control group. Twenty-one patients (21 eyes) who met the criteria were randomly assigned to 11 patients (11 eyes) in the treatment group and 10 eyes (10 eyes) in the control group. Twenty-five patients (40 eyes) in the treatment group were treated with laser combined with traditional Chinese medicine, while 25 patients (38 eyes) in the control group were treated only with laser. The clinical effective rates of the two groups were observed. All the patients were followed up for 15 days and 30 days after the treatment. The visual acuity, fundus changes and fundus angiography were observed before and after treatment in two groups, and the therapeutic effects of the two groups were analyzed. And draw a corresponding conclusion. Results: (1) the effective rate of retinal hemorrhage, exudation and microaneurysm absorption was 92.5% in the treatment group and 77.8% in the control group after one month treatment, there was significant difference between the two groups (P0.05). (2) the effective rate of macular edema disappearance was 94.1% in the treatment group and 71.4% in the control group 1 month after treatment, there was significant difference between the two groups (P0.05). (3) 1 month after treatment, the total effective rate was 82.5% in the treatment group and 52.8% in the control group, and there was significant difference between the two groups (P0.05). Conclusion: the therapeutic effect of the treatment group is superior to that of the control group. The therapeutic effect of the treatment group on retinal hemorrhage and exudation absorption is significantly better than that of the control group. The macular edema absorption of the treatment group is one month after treatment. The disappearance of microaneurysms and the improvement of visual acuity were significantly better than those of the control group, which indicated that Chinese herbal medicine combined with laser therapy had some advantages in the treatment of diabetic retinopathy.
