【摘要】： 目的：探討局部復(fù)發(fā)鼻咽癌的臨床特點(diǎn)、治療方法以及調(diào)強(qiáng)放療聯(lián)合愛(ài)必妥治療局部復(fù)發(fā)鼻咽癌的療效和安全性。資料及方法：廣西醫(yī)科大學(xué)第一附屬醫(yī)院2007年12月至2009年10月收治的3例接受IMRT聯(lián)合愛(ài)必妥治療的局部復(fù)發(fā)鼻咽癌的臨床特點(diǎn)：(1)3例患者均為女性,年齡分別為38歲、53歲和39歲；(2)3例患者初治分期均為T3N0M0,1例行常規(guī)放療后7年復(fù)發(fā)；2例行調(diào)強(qiáng)放療后1年內(nèi)復(fù)發(fā)；復(fù)治分期分別為rT4N0M0、rT4N0M0、rT4N1aM0(3)治療方法：2例患者采用7野調(diào)強(qiáng),1例放療劑量GTV58Gy, CTV48Gy,后去外院行賽博刀治療,1例放療劑量GTV68Gy, CTV 58Gy,另1例采用9野調(diào)強(qiáng),GTV70Gy, CTV 60Gy; (7)3例患者均聯(lián)合愛(ài)必妥分子靶向治療,放療前1周開(kāi)始用藥,第1周400mg／m2,從第2周開(kāi)始250 mg／m2,至放療結(jié)束。結(jié)果：3例患者均能耐受治療,短期內(nèi)復(fù)查MRI,病灶均較治療前明顯好轉(zhuǎn),顱神經(jīng)癥狀明顯改善。結(jié)論：調(diào)強(qiáng)放療聯(lián)合分子靶向治療可改善局部復(fù)發(fā)鼻咽癌患者的局部癥狀,近期療效滿意,毒副反應(yīng)可耐受。
[Abstract]:Objective: to investigate the clinical features, treatment methods and efficacy and safety of intensity-modulated radiotherapy (IMRT) combined with Ebitol in the treatment of locally recurrent nasopharyngeal carcinoma (RNPC). Materials and methods: from December 2007 to October 2009, 3 patients with locally recurrent nasopharyngeal carcinoma treated with IMRT combined with Abitol were treated in the first affiliated Hospital of Guangxi Medical University: (1) the 3 patients were all female, aged 38 years, respectively. 53 and 39 years; (2) all of the 3 patients were diagnosed as T3N0M0 stage, 1 case had recurrence after conventional radiotherapy for 7 years, 2 cases had recurrence within 1 year after intensity modulated radiotherapy, 2 cases had recurrence within 1 year after intensity modulated radiotherapy (IMRT). Two patients were treated with 7 field intensity modulation, one patient with radiotherapy dose of GTV58Gy, CTV48Gy, was treated with Cyber knife, one case with radiotherapy dose of GTV68Gy, CTV 58 Gy, the other case with 9 field intensity modulation, GTV70Gy,. CTV 60 Gy; (7) all of the 3 patients were treated with Ebitol molecular targeted therapy. The drug was taken 1 week before radiotherapy and 400 mg / m2 from week 2 to the end of radiotherapy. Results: all the 3 patients were able to tolerate the treatment. The focus of MRI, was obviously improved and the symptoms of cranial nerve were improved in a short time. Conclusion: intensity modulated radiotherapy combined with molecular targeting therapy can improve the local symptoms of patients with locally recurrent nasopharyngeal carcinoma.
