【摘要】：目的：觀察達(dá)明飲聯(lián)合羥苯磺酸鈣治療非增殖期糖尿病視網(wǎng)膜病變的臨床療效,探討其可能的作用機(jī)制。 方法：觀察NPDR患者36例(67眼),隨機(jī)分為治療組17例(32眼)和對照組19例(35眼)。治療組口服中藥達(dá)明飲及羥苯磺酸鈣,對照組口服羥苯磺酸鈣,連續(xù)服用30天。觀察服藥前后視力、眼底、眼底熒光血管造影、血流變及血脂的變化。 結(jié)果：(1).視力檢查比較,兩組自身治療前后對比有統(tǒng)計(jì)學(xué)意義(P0.05),用藥后組間比較有統(tǒng)計(jì)學(xué)意義(P0.05)。表明兩組均能提高DR患者的視力,治療組優(yōu)于對照組。(2).眼底熒光血管造影比較,治療組總有效率為94.7%,對照組總有效率為74.3%,治療后組間比較有統(tǒng)計(jì)學(xué)意義(P0.05),表明兩組眼底明可以明顯改善患者的眼底,且治療組優(yōu)于對照組。(3)血液流變學(xué)及血脂的比較,自身治療前后對比有統(tǒng)計(jì)學(xué)意義(P0.05),治療后組間比較有顯著差異(P0.01或P0.05),表明兩組均能改善血液流變學(xué),降低血脂,治療組改善血液流變學(xué)及血脂優(yōu)于對照組。 結(jié)論：達(dá)明飲聯(lián)合羥苯磺酸鈣治療NPDR,能提高視力,改善視網(wǎng)膜微循環(huán)；聯(lián)合用藥明顯優(yōu)于單用羥苯磺酸鈣的療效；作用機(jī)制可能與改善血液流變學(xué)及血脂、改善眼底微循環(huán),提高視網(wǎng)膜對缺血缺氧的耐受性有關(guān)。
[Abstract]:Objective: to observe the clinical effect of Daming decoction combined with calcium dobesilate in the treatment of non proliferative diabetic retinopathy and explore its possible mechanism. Methods: 36 patients (67 eyes) with NPDR were randomly divided into treatment group (n = 17) and control group (n = 19) (35 eyes). The treatment group took Daming decoction and calcium dobesilate, while the control group took calcium dobesilate for 30 days. The changes of visual acuity, fundus fluorescein angiography, hemorheology and blood lipid were observed. Results: (1). The visual acuity of the two groups before and after treatment was statistically significant (P0.05), and the comparison between the two groups was statistically significant after medication (P0.05). The results showed that both groups could improve the visual acuity of DR patients, and the treatment group was superior to the control group. (2). Fundus fluorescein angiography showed that the total effective rate in the treatment group was 94.77.The total effective rate in the control group was 74.3. There was significant difference between the two groups after treatment (P0.05). And the treatment group is better than the control group. (3) the comparison of hemorheology and blood lipid was statistically significant before and after treatment (P0.05), and there was significant difference between the groups after treatment (P0.01 or P0.05). The results showed that both groups could improve hemorheology and reduce blood lipids, while the treatment group was superior to the control group in improving hemorheology and blood lipids. Conclusion: Daming decoction combined with calcium dobesilate can improve visual acuity and improve retinal microcirculation in NPDR, and the combined therapy is better than calcium dobesilate alone. The mechanism may be related to the improvement of hemorheology and blood lipid, the improvement of fundus microcirculation and the tolerance of retina to ischemia and hypoxia.
