濕性年齡相關(guān)性黃斑變性疾病的視網(wǎng)膜電位特征

【摘要】:目的:視網(wǎng)膜電圖(electroretinogram, ERG)是一種客觀反應(yīng)視網(wǎng)膜功能的無(wú)創(chuàng)性檢查。比較濕性年齡相關(guān)性黃斑變性(wet-AMD)患者的ERG檢查中a、b波振幅、潛時(shí)與正常人間的差異,明確濕性AMD患者的ERG特點(diǎn)。同時(shí)比較不同程度AMD病變患者ERG的a、b波振幅和潛時(shí)差異,觀察ERG各參數(shù)與病變程度間的關(guān)系。 方法:對(duì)照組12例(14眼)和AMD患者組16人(18眼)均行矯正視力、裂隙燈、ERG及眼底檢查,AMD患者同時(shí)行眼視網(wǎng)膜血管造影(FFA)、眼脈絡(luò)膜血管造影(ICG)及光學(xué)相干斷層掃描(OCT)檢查,并根據(jù)OCT結(jié)果將AMD患者分3組,分別記錄AMD患者與正常組及AMD患者組內(nèi)暗適應(yīng)和明適應(yīng)3.0時(shí)a、b波的振幅及潛時(shí),用SPSS17.00軟件分析各組參數(shù)間的差異性。 結(jié)果:AMD組與對(duì)照組比較暗適應(yīng)3.0時(shí)a波振幅P值為0.002,潛時(shí)P值為0.084,b波振幅P值為0.000,潛時(shí)P值為0.054;明適應(yīng)3.0時(shí)a波振幅P值為0.000,潛時(shí)P值為0.000,b波振幅P值為0.000,潛時(shí)P值為0.059。AMD患者根據(jù)OCT結(jié)果分組比較:暗適應(yīng)3.0時(shí)a波振幅P值為0.555,潛時(shí)P值為0.432,b波振幅P值為0.291,潛時(shí)P值為0.385;明適應(yīng)3.0時(shí)a波振幅P值為0.236,潛時(shí)P值為0.001,b波振幅P值為0.045,潛時(shí)P值為0.138。 結(jié)論:與正常人相比濕性AMD患者a、b波振幅明顯降低,這與視錐、視桿系統(tǒng)損傷相關(guān)。明適應(yīng)ERG是在濕性AMD病變中反應(yīng)較敏感,,但能否將ERG作為濕性AMD病變分級(jí)的指標(biāo)需進(jìn)一步研究。
[Abstract]:Objective: electroretinogram (electroretinogram, ERG) is a non-invasive examination of retinal function. To compare the amplitude and latent time of ERG in ERG of patients with wet age-related macular degeneration (wet-AMD), and to determine the characteristics of ERG in the patients with wet age related macular degeneration (wet-AMD). At the same time, the amplitude and latent time of ERG in patients with different degrees of AMD lesions were compared, and the relationship between the parameters of ERG and the degree of lesion was observed. Methods: visual acuity, slit lamp, ERG and fundus examination were performed in 12 patients (14 eyes) in the control group and 16 eyes (18 eyes) in the AMD group. Retinal angiography (FFA), choroidal angiography (ICG) and optical coherence tomography (OCT) were performed in AMD patients. According to the results of OCT, the patients with AMD were divided into three groups. The amplitude and latent time of dark adaptation and light adaptation were recorded in AMD group, normal group and AMD group respectively. The difference of parameters in each group was analyzed by SPSS17.00 software. Results: compared with the control group, the amplitude of a wave (P = 0.002), latent time (P = 0.084) and latent time (P = 0.054) in AMD group were compared with those in control group. A wave amplitude P was 0.000, latent P value was 0.000b wave amplitude P value was 0.0000.000, latent P value of 0.059.AMD patients was compared according to OCT results: a wave amplitude P value was 0.555 at dark adaptation 3.0, latent P value was 0.432b wave amplitude P value was 0.291, latent P value was 0.385; The amplitude of a wave P is 0.236, the latent P value is 0.001kb wave amplitude P is 0.045, and the latent P value is 0.138. Conclusion: the amplitude of AGB wave in wet AMD patients is significantly lower than that in normal subjects, which is related to the injury of optic cone and rod system. Light adaptive ERG is more sensitive in the pathological changes of AMD, but whether ERG can be used as the index of pathological grading of wet AMD needs further study.
