【摘要】：目的:探討阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者血漿褪黑激素水平與腦電覺醒指數(shù)、呼吸暫停低通氣指數(shù)(AHI)及最低血氧飽和度之間的關(guān)系;探討褪黑激素水平在OSAHS患者白天嗜睡程度評估中的價值。 方法:選取2007年～2009年在我院行多導(dǎo)睡眠監(jiān)測(PSG)的患者87例(其中17例為對照組,70例為OSAHS患者)。早晨6點取靜脈血2ml,用酶聯(lián)免疫分析方法檢測血漿褪黑激素水平。參照美國睡眠醫(yī)學(xué)會標(biāo)準(zhǔn),根據(jù)PSG監(jiān)測結(jié)果將OSAHS患者分為輕、中、重、極重度四組,其中輕度11例,中度14例,重度19例,極重度26例;對照組17例根據(jù)PSG及有無打鼾分為正常對照組及單純鼾癥對照組,其中正常對照組11例,單純鼾癥對照組6例。 結(jié)果:①OSAHS患者與對照組相比,血漿褪黑激素水平升高有統(tǒng)計學(xué)意義(P0.05);②血漿褪黑激素水平與腦電覺醒指數(shù)及AHI值呈正相關(guān),與最低血氧飽和度呈負(fù)相關(guān),且均有統(tǒng)計學(xué)意義(P0.05)。③血漿褪黑激素水平與ESS評分呈正相關(guān),經(jīng)統(tǒng)計學(xué)檢驗無統(tǒng)計學(xué)意義(P0.05)。 結(jié)論:OSAHS患者血漿褪黑激素水平升高,且褪黑激素水平與OSAHS患病程度有密切聯(lián)系;褪黑激素水平可反映患者白天嗜睡程度,對OSAHS患者白天嗜睡程度評估可能有重要價值。
[Abstract]:Objective: to investigate the relationship between plasma melatonin level and EEG arousal index, apnea hypopnea index (AHI) and minimum oxygen saturation in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). To investigate the value of melatonin level in daytime sleepiness in OSAHS patients. Methods: 87 patients with polysomnography (PSG) were selected from 2007 to 2009 in our hospital (17 cases were control group, 70 cases were OSAHS patients). Plasma melatonin levels were detected by enzyme linked immunosorbent assay (Elisa). According to the standard of American Sleep Medical Association, OSAHS patients were divided into four groups according to the results of PSG monitoring: mild, moderate, severe, and extremely severe, 11 mild, 14 moderate, 19 severe and 26 extremely severe. According to PSG and snoring, 17 cases in control group were divided into normal control group and simple snoring control group, including 11 normal control group and 6 simple snoring control group. Results compared with the control group, the plasma melatonin level was significantly increased (P0.05). The plasma melatonin level was positively correlated with the EEG arousal index and AHI, and negatively correlated with the lowest oxygen saturation. There was a positive correlation between plasma melatonin level and ESS score (P0.05), but there was no statistical significance by statistical test (P0.05). Conclusion the plasma melatonin level is increased in patients with OSAHS and melatonin level is closely related to the severity of OSAHS. Melatonin level may reflect daytime sleepiness in patients with OSAHS and may be of great value in the evaluation of daytime sleepiness in OSAHS patients.
