晶狀體半脫位手術(shù)226眼臨床觀察

【摘要】：目的觀察超聲乳化晶體摘除聯(lián)合張力環(huán)并Ⅰ期人工晶體植入術(shù)治療晶狀體半脫位的效果。方法①晶體半脫位1個(gè)象限的患者30例(43眼),直接行超聲乳化晶體核,吸凈皮質(zhì),用旋轉(zhuǎn)法植入(CTR),囊袋內(nèi)植入人工晶體。②晶體半脫位于1~2個(gè)象限的患者111例(152眼),先用旋轉(zhuǎn)法植入CTR,再超聲乳化晶體核,吸凈皮質(zhì),囊袋內(nèi)植入人工晶體。③晶狀體半脫位2個(gè)象限的患者21例(31眼),應(yīng)用囊袋固定型張力環(huán),采用聚丙烯縫線將張力環(huán)縫合固定于鞏膜,拉緊張力環(huán)縫線至晶狀體囊袋居正中,抽吸晶狀體皮質(zhì),植入折疊式人工晶狀體。結(jié)果 162例226眼患者,術(shù)后隨訪1~12個(gè)月,人工晶狀體基本保持正位,裸眼視力≥0.5者199眼(88!)。結(jié)論超聲乳化晶體摘除聯(lián)合張力環(huán)并人工晶體植入,是晶體半脫位的首選治療方法。脫位范圍2個(gè)象限的患者可直接植入張力環(huán),若脫位范圍2個(gè)象限,則應(yīng)縫線固定張力環(huán)。該手術(shù)方式可有效防止術(shù)后人工晶狀體偏位的發(fā)生,大大提高患者的術(shù)后視力。
[Abstract]:Objective to observe the effect of phacoemulsification combined with tension ring and intraocular lens implantation in the treatment of subluxation of lens. Methods (1) 30 patients (43 eyes) with subluxation of lens in one quadrant were treated with phacoemulsification of lens nucleus and cortical aspiration. 111 patients (152 eyes) with intraocular lens subluxation in 1 ~ 2 quadrants were implanted in (CTR), capsule bag by rotation method. CTR, was implanted with phacoemulsification to remove the cortex. Twenty-one patients (31 eyes) with subluxation of intraocular lens (IOL. 3) were implanted in the capsule bag. The tension loop was sutured to the sclera by polypropylene suture, and the tension loop was tightened to the middle of the capsule bag. Pump the lens cortex and implant the foldable intraocular lens. Results 162 cases (226 eyes) were followed up for 1 ~ 12 months. Intraocular lens remained in positive position basically. The naked visual acuity was better than 0.5 in 199 eyes (88!). Conclusion phacoemulsification combined with tension ring and intraocular lens implantation is the first choice for the treatment of lens subluxation. Patients with dislocation range 2 quadrants can directly implant tension rings, and if dislocation ranges 2 quadrants, suture tension rings should be fixed. This method can effectively prevent the postoperative IOL deviation and greatly improve the postoperative visual acuity.
