年齡相關(guān)性白內(nèi)障的高階像差

【摘要】: 目的:觀察三種不同類型的年齡相關(guān)性白內(nèi)障的高階像差,對(duì)不同類型年齡相關(guān)性白內(nèi)障患者的高階像差進(jìn)行量化分析并比較其變化特點(diǎn)。為進(jìn)一步改善白內(nèi)障術(shù)后的視覺(jué)質(zhì)量提供理論依據(jù)。 方法:回顧性病例分析。選擇不同類型的年齡相關(guān)性白內(nèi)障患者109例163眼,年齡44歲~81歲,平均(66.45±9.421)歲,其中女性72例(66.06%)。入選標(biāo)準(zhǔn)為患眼最佳矯正視力"g0.3logMAR(等值球鏡度數(shù)在-3.0~+3.0D之間),排除有其他任何眼部疾病、眼部手術(shù)史及晶狀體混濁嚴(yán)重致使Tracey像差儀不能準(zhǔn)確測(cè)量的患者。根據(jù)晶狀體混濁分級(jí)系統(tǒng)Ⅲ(lens opacities classification systemⅢ, LOCSⅢ)進(jìn)行白內(nèi)障分類和分組。其中皮質(zhì)性59眼,核性62眼,后囊下性42眼,30例30眼透明晶狀體作為對(duì)照組,使用Tracey像差儀(Tracey wavefront aberrometer, Tracey Technologies Inc., Houston, USA)分別測(cè)量6.0mm瞳孔直徑下各組的高階像差。Tracey系統(tǒng)計(jì)算和分析共6階27項(xiàng)Zernike系數(shù)和均方根值(root mean square, RMS),比較白內(nèi)障組和對(duì)照組的結(jié)果差異,分析并比較不同類型的年齡相關(guān)性白內(nèi)障高階像差的變化特點(diǎn)。 結(jié)果:6.0mm瞳孔直徑下,以晶狀體皮質(zhì)混濁為主的皮質(zhì)性白內(nèi)障組總高階像差為(0.701±0.230)μm,高階主導(dǎo)像差主要以彗差(0.438±0.213)μm為主,其次為三葉草(0.357±0.203)μm,在各高階像差中,除球差(P=.781)、五葉草(P=.384)、六葉草(P=.390)外,彗差(P=.000)、高級(jí)散光(P=.002)、三葉草(P=.002)、四葉草(P=.005)均有統(tǒng)計(jì)學(xué)意義。以晶狀體核混濁為主的核性白內(nèi)障組總高階像差為(1.101±0.303)μm,高階主導(dǎo)像差主要以球差(0.497±0.231)μm為主,其次為彗差(0.466±0.291)μm,在各高階像差中,球差(P=.000)、彗差(P=.002)、高級(jí)散光(P=.003)、三葉草(P=.008)、四葉草(P=.002)、五葉草(P=.006)、六葉草(P=.007)均有統(tǒng)計(jì)學(xué)意義。以晶狀體后囊膜下混濁為主的后囊下性白內(nèi)障組總高階像差為(0.645±0.261)μm,高階主導(dǎo)像差主要以三葉草(0.335±0.142)μm為主,其次為彗差(0.294±0.178)μm,在各高階像差中,球差(P=.008)、彗差(P=.004)、高級(jí)散光(P=.000)、三葉草(P=.000)、四葉草(P=.008)、五葉草(P=.003)、六葉草(P=.001)均有統(tǒng)計(jì)學(xué)意義。 結(jié)論:不同類型的年齡相關(guān)性白內(nèi)障產(chǎn)生不同特點(diǎn)的高階像差,皮質(zhì)性白內(nèi)障主要引起慧差的改變,核性白內(nèi)障主要使球差增加,后囊下性白內(nèi)障的三葉草明顯增加。白內(nèi)障患者主視視覺(jué)質(zhì)量的下降與各階像差不同程度的改變相關(guān)。
