眼表燒傷患者結(jié)膜囊?guī)Ь治?/H1>

【摘要】：目的 了解燒傷狀態(tài)下結(jié)膜囊的帶菌情況，為其預(yù)防性使用抗感染藥品提供依據(jù)。 方法 對我院眼表燒傷患者（26只燒傷眼）及自愿者（25只正常眼）的結(jié)膜囊進行細菌培養(yǎng)和藥物敏感試驗，并對其結(jié)果進行分析。 結(jié)果 眼表燒傷患者患眼結(jié)膜囊細菌陽性率為53.85％（14/26），對照組結(jié)膜囊細菌陽性率為56.00%(14/25)，兩組比較差異無統(tǒng)計學(xué)意義(χ~2=0.024，P=0.877)。燒傷眼（26例）與正常眼（25例）均培養(yǎng)出2個菌種，5個菌株。正常眼所檢出的14例陽性菌株中，革蘭氏陽性球菌占92.86%，其次為革蘭氏陽性桿菌，，占7.14%。陽性菌株中表皮葡萄球菌9株(64.29%)，頭狀葡萄球菌2株(14.29％)。燒傷眼結(jié)膜囊檢出的14例陽性菌株，主要由革蘭氏陽性球菌構(gòu)成（92.86%），其次為革蘭氏陽性桿菌（占7.14%）。陽性菌株中表皮葡萄球菌6株(42.86%)，溶血葡萄球菌5株(35.74％)。2組結(jié)膜囊優(yōu)勢菌種均以表皮葡萄球菌為主。燒傷眼表皮葡萄球菌占總檢出陽性菌株的42.86%，較正常組的64.29%低，差異無統(tǒng)計學(xué)意義(P=0.449)；燒傷眼溶血性葡萄球菌陽性檢出率明顯高于正常眼，兩組差異有統(tǒng)計學(xué)意義(P=0.041)。 結(jié)論 燒傷眼與正常眼結(jié)膜囊內(nèi)的細菌為常居菌。兩組細菌陽性檢出率、菌種數(shù)、革蘭分類的構(gòu)成比無明顯差異。但較于正常眼，燒傷眼結(jié)膜囊溶血葡萄球菌陽性率明顯增高。燒傷患者治療過程中，應(yīng)常規(guī)進行結(jié)膜囊細菌培養(yǎng)，并根據(jù)其結(jié)果選擇敏感抗生素防止感染。
[Abstract]:Objective to investigate the bacterial infection of conjunctival sac under burn condition and to provide evidence for prophylactic use of anti-infective drugs. Methods bacterial culture and drug sensitivity test were carried out in conjunctival sac of 26 burn eyes and 25 normal eyes in our hospital. Results the bacterial positive rate of conjunctival sac was 53.85% (14 / 26) in patients with ocular surface burn and 56.00% (14 / 25) in control group. Two strains and five strains were cultured in burn eyes (26 cases) and normal eyes (25 cases). Among the 14 positive strains detected in normal eyes, Gram positive cocci accounted for 92.86%, followed by Gram-positive bacilli (7.14%). 9 strains (64.29%) were Staphylococcus epidermidis and 2 strains (14.29%) were Staphylococcus cephalis. The 14 positive strains were mainly composed of Gram-positive cocci (92.86%), followed by Gram-positive bacilli (7.14%). Among the positive strains, Staphylococcus epidermidis 6 strains (42.86%), Staphylococcus haemolyticus 5 strains (35.74%) were dominant bacteria in conjunctival sac of group 2, Staphylococcus epidermidis was the dominant bacteria. The positive rate of Staphylococcus epidermidis in burn eyes was 42.86%, which was lower than that in normal group (64.29%), but the positive rate of haemolytic staphylococcus in burn eyes was significantly higher than that in normal eyes (P0. 041). Conclusion bacteria in conjunctival sac of burn eyes and normal eyes are common bacteria. There was no significant difference in the positive rate of bacteria, the number of bacteria and the composition ratio of Gram classification between the two groups. However, the positive rate of staphylococcus conjunctiva hemolysis in burn eyes was significantly higher than that in normal eyes. The bacterial culture of conjunctival sac should be performed regularly during the treatment of burn patients, and sensitive antibiotics should be selected according to the results to prevent infection.
