不同切口設計的白內障超聲乳化術后角膜變化的研究

【摘要】:目的:通過眼科常規(guī)檢查及角膜地形圖儀觀察兩種不同設計的切口白內障手術前后視力、散光及角膜形態(tài)的變化。 方法:收集在我院接受白內障超聲乳化吸除聯合IOL植人術的患者40例(46眼),隨機分為A、B兩組。A組作鞏膜隧道自閉式切口,B組作透明角膜切口,兩組位置均在上方(12:00位),大小為3.0mm,兩組核硬度均在Ⅱ-Ⅲ級之間。采用眼科常規(guī)檢查及角膜地形圖儀觀察白內障手術前,術后1天、術后1月、術后3月的裸眼視力、散光及角膜前表面的形態(tài)變化。 結果:兩組手術前的視力比較,差異無統計學意義(PO.05),術后1天A組的裸眼視力好于B組**(P0.05)。術后1天、1月、3月兩組的裸眼視力均較術前有所提高*(P0.05)。術后1月、3月的裸眼視力兩組比較差別不明顯,差異無統計學意義(P0.05)。兩組術前的散光比較,差異無統計學意義(PO.05),術后1天、1月、3月A組的散光度數小于B組,兩組差異有統計學意義**(P0.05)。而術后三個月和術前比較散光有所減小,兩組差異有統計學意義*(P0.05)。角膜地形圖兩組術后1天上方切口所在的徑線較手術前變陡峭,角膜彩色色碼圖較術前向暖色調變化。隨著時間的推移,兩組散光均有所減小。切口對應處(12點位)周邊部的角膜變得相對平坦,角膜曲率變小,角膜彩色色碼圖向冷色調變化,其余部分彩色色碼圖無明顯變化。術后1月、3月兩組患者的變化不明顯。 結論:1.鞏膜隧道切口對角膜形態(tài)的影響較小、散光較小。2.鞏膜隧道切口具有同透明角膜切口相同的優(yōu)點,術后視力恢復較好、較安全。3.鞏膜隧道切口術后早期的視力好于角膜切口組,遠期兩組的差別不明顯。4.選擇在角膜最大屈光力的徑線上作切口對所在子午線的角膜曲率有松解作用,能在一定程度上矯正部分散光。
