【摘要】：目的觀察白內(nèi)障合并角膜散光患者植入Acrysof toric人工晶狀體術(shù)后的視覺質(zhì)量以及toric晶狀體的眼內(nèi)穩(wěn)定性。 方法收集2009年10月至2010年9月在我院行白內(nèi)障摘除聯(lián)合Acrysof toric人工晶狀體植入的患者60例80眼作為觀察組,依據(jù)人工晶狀體型號(hào)分為T3組,T4組,T5a組及T5b組,術(shù)后3個(gè)月觀察患者的裸眼視力,最佳矯正視力,殘余散光,對(duì)比敏感度,波前像差,晶狀體的旋轉(zhuǎn)度數(shù)、偏心值及傾斜度數(shù)。選取同期植入Acrysof Natural人工晶狀體的患者45例75眼作為對(duì)照組。 結(jié)果術(shù)后3個(gè)月觀察組70眼(87.5%)的UCVA≥0.5,對(duì)照組51眼(68%)的UCVA≥0.5,兩組間比較差別有統(tǒng)計(jì)學(xué)意義(χ2=12.18,P0.05)；T3,T4,T5組術(shù)后視力均較術(shù)前改善,T5b組視力低于其余三組(χ2=20.37,P0.05)。觀察組術(shù)后殘余散光為(0.51±0.78)D,明顯低于對(duì)照組(1.44±0.72)D(t=10.54,P0.05)；T5b組術(shù)后殘余散光為(1.69±0.68)D,與T3,T4及T5a組比較均有統(tǒng)計(jì)學(xué)差異。觀察組各個(gè)空間頻率的對(duì)比敏感度均高于對(duì)照組,低階像差及彗差低于對(duì)照組。T3,T4,T5a組對(duì)比敏感度高于T5b組,低階像差及彗差低于T5b組。toric人工晶狀體平均旋轉(zhuǎn)(3.27±1.72)°,67眼(83.75%)旋轉(zhuǎn)≤5°,79眼(98.75%)旋轉(zhuǎn)≤10°；偏心值為(0.28±0.06)mm；傾斜度數(shù)為(2.08±0.14)。 結(jié)論Acrysof toric人工晶狀體植入矯正散光療效可靠,術(shù)后表現(xiàn)出良好的旋轉(zhuǎn)及居中穩(wěn)定性,患者獲得較滿意的視覺質(zhì)量,是矯正白內(nèi)障合并角膜散光的可靠的手術(shù)方式。
[Abstract]:Objective to observe the visual quality and intraocular stability of Acrysof toric intraocular lens (IOL) implantation in cataract patients with corneal astigmatism. Methods from October 2009 to September 2010, 60 patients (80 eyes) with cataract extraction and Acrysof toric intraocular lens implantation in our hospital were selected as observation group. According to the type of intraocular lens, they were divided into T3 group, T4 group, T5a group and T5b group.
