特發(fā)性黃斑裂孔內(nèi)界膜手術(shù)對視覺功能影響的研究

【摘要】:目的:檢測特發(fā)性黃斑裂孔手術(shù)方法中,黃斑內(nèi)界膜剝除與否對術(shù)眼視覺功能的影響。探討特發(fā)性黃斑裂孔手術(shù)治療的安全有效方法及其適應(yīng)癥。 方法:明視負(fù)波(PhNR)是一項(xiàng)檢測神經(jīng)節(jié)細(xì)胞和視神經(jīng)纖維損害的功能變化的有效手段。對連續(xù)收治的病程在6個月以內(nèi)的Ⅲ、Ⅳ期特發(fā)性黃斑裂孔,行經(jīng)睫狀體平坦部三通道玻璃體切除聯(lián)合無染色劑輔助的黃斑區(qū)視網(wǎng)膜內(nèi)界膜剝除術(shù),術(shù)后眼內(nèi)氣體填充、面向下體位。除常規(guī)眼科檢查外,術(shù)前及術(shù)后3個月檢測記錄藍(lán)色背景紅色刺激光條件下視網(wǎng)膜電流圖(ERG),比較手術(shù)前、手術(shù)后視網(wǎng)膜電流圖中α、β波和明視負(fù)波(PhNR)的振幅變化。對同期收治的Ⅱ期特發(fā)性黃斑裂孔行單純玻璃體切除術(shù),切除玻璃體后皮質(zhì),術(shù)后眼內(nèi)氣體填充、面向下體位。相同方法檢測、比較手術(shù)前、手術(shù)后視網(wǎng)膜電流圖α、β波和明視負(fù)波(PhNR)的振幅變化。隨訪6~12個月。觀察黃斑內(nèi)界膜剝除手術(shù)前后視覺功能的影響,黃斑內(nèi)界膜未剝除手術(shù)前后視覺功能的變化。統(tǒng)計(jì)學(xué)方法:所有數(shù)據(jù)均采用SPSS13.0軟件進(jìn)行數(shù)據(jù)分析。手術(shù)前后LogMAR視力對比采用Wilcoxon配對秩和檢驗(yàn);手術(shù)前后ERG各波振幅對比采用Wilcoxon Signed Ranks Test。以P0.05為有統(tǒng)計(jì)學(xué)意義。結(jié)果:對連續(xù)收治的Ⅲ、Ⅳ期特發(fā)性黃斑裂孔32例(32眼),術(shù)前平均裂孔直徑375.6μm(352μm~445μm),手術(shù)后裂孔解剖閉合30眼(30/32,93.6%),平均最佳矯正視力(BCVA)提高0.24(術(shù)前0.10±0.05,術(shù)后0.34±0.17),手術(shù)后PhNR平均振幅值較手術(shù)前下降12.1μν(術(shù)前33.6±3.9μν,術(shù)后21.5±6.5μν,P0.05),α、β波振幅略有升高,但無統(tǒng)計(jì)學(xué)意義(P0.05)。其中40%病人術(shù)后短期內(nèi)閉眼時眼前出現(xiàn)圖像閃爍感,術(shù)后注視小視標(biāo)時自覺閃爍感。Ⅱ期特發(fā)性黃斑裂孔26例(26眼),術(shù)前平均裂孔直徑258.4μm(230μm~292μm),手術(shù)后裂孔解剖閉合25眼(25/26,96.2%),平均BCVA提高0.31(術(shù)前0.33±0.10,術(shù)后0.64±0.19),手術(shù)后PhNR平均振幅值較手術(shù)前下降1.0μν(術(shù)前33.9±2.0μν,術(shù)后32.9±2.1μν,P0.05),α、β波振幅略有升高,但無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:特發(fā)性黃斑裂孔視網(wǎng)膜內(nèi)界膜剝除術(shù)使Ⅲ、Ⅳ期特發(fā)性黃斑裂孔手術(shù)后裂孔閉合、視力明顯提高,手術(shù)后視覺電生理存在一定的視網(wǎng)膜內(nèi)層功能的損害,但與其在裂孔閉合后視力明顯提高的意義相比影響甚小。而Ⅱ期特發(fā)性黃斑裂孔無需進(jìn)行內(nèi)界膜剝除術(shù),即可獲得良好的裂孔閉合率及視覺功能質(zhì)量。
