特發(fā)性黃斑裂孔內界膜手術對視覺功能影響的研究

【摘要】:目的:檢測特發(fā)性黃斑裂孔手術方法中,黃斑內界膜剝除與否對術眼視覺功能的影響。探討特發(fā)性黃斑裂孔手術治療的安全有效方法及其適應癥。 方法:明視負波(PhNR)是一項檢測神經節(jié)細胞和視神經纖維損害的功能變化的有效手段。對連續(xù)收治的病程在6個月以內的Ⅲ、Ⅳ期特發(fā)性黃斑裂孔,行經睫狀體平坦部三通道玻璃體切除聯(lián)合無染色劑輔助的黃斑區(qū)視網膜內界膜剝除術,術后眼內氣體填充、面向下體位。除常規(guī)眼科檢查外,術前及術后3個月檢測記錄藍色背景紅色刺激光條件下視網膜電流圖(ERG),比較手術前、手術后視網膜電流圖中α、β波和明視負波(PhNR)的振幅變化。對同期收治的Ⅱ期特發(fā)性黃斑裂孔行單純玻璃體切除術,切除玻璃體后皮質,術后眼內氣體填充、面向下體位。相同方法檢測、比較手術前、手術后視網膜電流圖α、β波和明視負波(PhNR)的振幅變化。隨訪6~12個月。觀察黃斑內界膜剝除手術前后視覺功能的影響,黃斑內界膜未剝除手術前后視覺功能的變化。統(tǒng)計學方法:所有數(shù)據(jù)均采用SPSS13.0軟件進行數(shù)據(jù)分析。手術前后LogMAR視力對比采用Wilcoxon配對秩和檢驗;手術前后ERG各波振幅對比采用Wilcoxon Signed Ranks Test。以P0.05為有統(tǒng)計學意義。結果:對連續(xù)收治的Ⅲ、Ⅳ期特發(fā)性黃斑裂孔32例(32眼),術前平均裂孔直徑375.6μm(352μm~445μm),手術后裂孔解剖閉合30眼(30/32,93.6%),平均最佳矯正視力(BCVA)提高0.24(術前0.10±0.05,術后0.34±0.17),手術后PhNR平均振幅值較手術前下降12.1μν(術前33.6±3.9μν,術后21.5±6.5μν,P0.05),α、β波振幅略有升高,但無統(tǒng)計學意義(P0.05)。其中40%病人術后短期內閉眼時眼前出現(xiàn)圖像閃爍感,術后注視小視標時自覺閃爍感。Ⅱ期特發(fā)性黃斑裂孔26例(26眼),術前平均裂孔直徑258.4μm(230μm~292μm),手術后裂孔解剖閉合25眼(25/26,96.2%),平均BCVA提高0.31(術前0.33±0.10,術后0.64±0.19),手術后PhNR平均振幅值較手術前下降1.0μν(術前33.9±2.0μν,術后32.9±2.1μν,P0.05),α、β波振幅略有升高,但無統(tǒng)計學意義(P0.05)。結論:特發(fā)性黃斑裂孔視網膜內界膜剝除術使Ⅲ、Ⅳ期特發(fā)性黃斑裂孔手術后裂孔閉合、視力明顯提高,手術后視覺電生理存在一定的視網膜內層功能的損害,但與其在裂孔閉合后視力明顯提高的意義相比影響甚小。而Ⅱ期特發(fā)性黃斑裂孔無需進行內界膜剝除術,即可獲得良好的裂孔閉合率及視覺功能質量。
