【摘要】：目的對(duì)比觀察Ex-PRESS引流釘植入術(shù)與小梁切除術(shù)治療開(kāi)角型青光眼(open angle glaucoma,OAG)的早期療效與安全性。方法回顧性分析在我院同一時(shí)期接受Ex-PRESS引流釘(P50)植入術(shù)(29例33眼,Ex-PRESS組)與小梁切除術(shù)(28例36眼,小梁組)患者的臨床資料,統(tǒng)計(jì)兩組患者的病種構(gòu)成、理論住院天數(shù)、并發(fā)癥發(fā)生率、出入院時(shí)的視力和眼壓變化情況,統(tǒng)計(jì)分析兩種手術(shù)治療OAG的短期療效與安全性。結(jié)果 Ex-PRESS組和小梁組患眼中男女比例分別為1.75∶1、1.77∶1(P0.05);年齡分別為(39±3)歲、(41±3)歲(P0.05)。Ex-PRESS組中單純的原發(fā)性開(kāi)角型青光眼(primary open angle glaucoma,POAG)患者較少(P0.05)。Ex-PRESS組和小梁組病例的理論住院時(shí)間分別為(3.12±0.33)d、(4.47±1.67)d (P0.05),Ex-PRESS組的理論住院時(shí)間較短。Ex-PRESS組和小梁組的并發(fā)癥發(fā)生率分別為0、8.33%(P0.05),Ex-PRESS組的并發(fā)癥較少。Ex-PRESS組和小梁組患者手術(shù)前后視力變化平均值分別為0.009、0.096(P0.05),Ex-PRESS組術(shù)后視力下降較少。Ex-PRESS組和小梁組患者手術(shù)前后眼壓下降值分別為(22.05±13.20)mmHg(1 kPa=7.5 mmHg)、(21.11±13.58)mmHg,兩組手術(shù)前后眼壓差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論Ex-PRESS引流釘植入術(shù)不僅能夠達(dá)到小梁切除術(shù)相同的早期治療效果,而且具有術(shù)后視力下降及并發(fā)癥較少、術(shù)后恢復(fù)快、住院時(shí)間短等優(yōu)點(diǎn),除適用于單純POAG外,尤其適用于各種繼發(fā)性O(shè)AG和伴有危險(xiǎn)因素POAG患者。
[Abstract]:Objective to compare the early efficacy and safety of Ex-PRESS drainage nail implantation and trabeculectomy in the treatment of open angle glaucoma (open angle glaucoma). Methods the clinical data of 29 patients (33 eyes of Ex-PRESS group) and 28 cases of 36 eyes of trabeculectomy (trabeculectomy group) received Ex-PRESS drainage nail (P50) and trabeculectomy in the same period were analyzed retrospectively. The incidence of complications, the changes of visual acuity and intraocular pressure (IOP) in hospital, and the short-term efficacy and safety of two kinds of operations in the treatment of OAG were analyzed statistically. Results in Ex-PRESS group and trabecular group, the ratio of male to female was 1.75: 1: 1.77: 1 (P0.05); the age was (39 鹵3) years, (41 鹵3) years (P0.05). There were fewer cases of primary open-angle (primary open angle glaucoma Poag in Ex-PRESS group (P0.05). Ex-PRESS group and trabecular group (P0.05). The incidence of complications in Ex-PRESS group and trabecular group was 0 ~ 8.33% (P0.05). The average visual acuity of Ex-PRESS group and trabecular group was 0.009 / 0.096 (P0.05), respectively. The postoperative visual acuity of Ex-PRESS group and trabecular group were 0.009 / 0.096 (P0.05). The incidence of complications in Ex-PRESS group was 0.33% (P0.05). The average visual acuity of Ex-PRESS group and trabecular group was 0.009 / 0.096 (P0.05). The postoperative visual acuity of Ex-PRESS group was less than that of Ex-PRESS group. The decrease of IOP was (22.05 鹵13.20) mmHg (1.kPa7.5mmHg), (21.11 鹵13.58) mmHg in Ex-PRESS group and trabecular group before and after operation, respectively. There was no significant difference in IOP between the two groups before and after operation (P0.05). Conclusion Ex-PRESS drainage nail implantation can not only achieve the same early treatment effect as trabeculectomy, but also has the advantages of reduced visual acuity, fewer complications, faster postoperative recovery and shorter hospital stay. It is suitable for simple Poag. It is especially suitable for patients with all kinds of secondary OAG and Poag with risk factors.
