【摘要】：【目的】比較配戴治療性角膜接觸鏡(TCL)前后Tono-pen眼壓計(jì)測量角膜病變患者眼壓測量的差異,分析治療性角膜接觸對(duì)Tono-pen眼壓計(jì)測量值的影響�！痉椒ā窟B續(xù)納入2012年4月至2012年10月我院就診的33例(33眼)角膜病變患者,包括大泡性角膜病變11眼、眼化學(xué)傷8眼、單皰病毒性角膜炎5眼、繼發(fā)性青光眼4眼、前部角膜營養(yǎng)不良2眼、穿透性角膜移植術(shù)后2眼、復(fù)發(fā)性胬肉1眼。應(yīng)用Tono-pen眼壓計(jì)測量患者未配戴TCL和配戴TCL兩種狀態(tài)下的眼壓,每種測量方式各重復(fù)測2次。用組內(nèi)相關(guān)系數(shù)ICC計(jì)算每一種測量方法的重復(fù)性,計(jì)算重復(fù)測量間差異的95%可信限;配對(duì)t檢驗(yàn)比較兩種眼壓測量方法的差異,計(jì)算差異的可信限95%LoA,并繪制Bland-Altman散點(diǎn)圖�！窘Y(jié)果】Tono-pen眼壓計(jì)直接重復(fù)測量的ICC為0.986,兩次測量的平均差值為(0.42±1.98)mmHg,95%可信限為-4.30~3.46 mmHg,其中90.9%的測量差值≤3 mmHg;配戴治療性角膜接觸鏡后重復(fù)測量的ICC為0.983,兩次測量的平均差值為(0.12±2.19)mmHg,95%可信限為-4.46~4.22 mmHg,90.9%的測量差值≤3 mmHg。配戴治療性角膜接觸鏡后Tono-pen眼壓測量值平均減小(0.68±3.39)mmHg,差異無統(tǒng)計(jì)學(xué)意義(P=0.257),兩種測量結(jié)果的一致性系數(shù)ICC為0.959,差值的95%可信限為-7.32~5.96 mmHg,其中70%測量的差值≤3 mmHg;在平均眼壓21 mmHg的患者中,82.4%的測量差值≤3 mmHg,在平均眼壓≥21 mmHg的患者中,56.3%的測量差值≤3 mmHg。【結(jié)論】Tono-pen眼壓計(jì)測量角膜病變患者的眼壓有較好的重復(fù)性,配戴治療性角膜接觸鏡對(duì)測量的重復(fù)性和總體的測量均值無影響。但配戴角膜接觸鏡前后眼壓測量值存在一定程度的差異,高眼壓者比正常眼壓者的差異大,臨床應(yīng)用時(shí)應(yīng)對(duì)測量結(jié)果進(jìn)行客觀的評(píng)價(jià)。
[Abstract]:[objective] to compare the difference of intraocular pressure (IOP) measurement between patients with keratopathy before and after wearing a therapeutic contact lens (TCL) with Tono-pen intraocular pressure (IOP). The effect of therapeutic corneal contact on Tono-pen IOP was analyzed. [methods] 33 cases (33 eyes) of keratopathy were included in our hospital from April 2012 to October 2012, including 11 eyes with macular keratopathy and 8 eyes with chemical injury. Herpes simplex keratitis in 5 eyes, secondary glaucoma in 4 eyes, anterior corneal dystrophy in 2 eyes, penetrating keratoplasty in 2 eyes, recurrent pterygium in 1 eye. The intraocular pressure (IOP) was measured with Tono-pen intraocular pressure meter under the condition of non-wearing TCL and TCL. The repeatability of each measurement method was calculated with the intra-group correlation coefficient ICC, and the 95% confidence limit of the difference between repeated measurements was calculated. Calculate the confidence limit of difference 95LoA, and draw Bland-Altman scatter plot.
