鼻咽癌中VEGF及其受體EGFR的表達(dá)及相關(guān)性的初步研究

【摘要】:背景和目的: 鼻咽癌是頭頸部最常見的惡性腫瘤,世界衛(wèi)生組織統(tǒng)計(jì)大約有80%的鼻咽癌發(fā)生與我國。VEGF(血管內(nèi)皮生長(zhǎng)因子)是目前已知最具活性的促血管生成因子之一,能促進(jìn)血管內(nèi)皮增殖,增加血管通透性,為血管內(nèi)皮細(xì)胞的生長(zhǎng)、遷移提供基質(zhì)條件,并且具有抑制腫瘤細(xì)胞凋亡的重要作用。EGFR(血管內(nèi)皮生長(zhǎng)因子受體)對(duì)VEGF具有高度親和性,二者特異結(jié)合后,促發(fā)細(xì)胞分裂信號(hào),引起細(xì)胞增殖,EGFR表達(dá)異常,可使正常細(xì)胞轉(zhuǎn)化成腫瘤細(xì)胞,并增加腫瘤的侵襲和轉(zhuǎn)移能力。本研究運(yùn)用免疫組化檢測(cè)57例鼻咽癌組織中的VEGF、EGFR的表達(dá),27例鼻咽部慢性炎癥組織中的VEGF、EGFR的表達(dá),研究VEGF和EGFR表達(dá)水平與臨床資料之間以及相互之間的關(guān)系。 材料和方法: 2009年-2010年在我院初診確定為鼻咽癌的鼻咽部活檢標(biāo)本57例。其中男性44例,女性13例;年齡為17-74歲;病理分型為非角化性癌未分化型19例,非角化性癌分化型30例,低分化鱗狀細(xì)胞癌8例。根據(jù)2002年AJCC標(biāo)準(zhǔn)57例中T1 27例,T2 17例,T3 8例,T4 5例;N0 16例,N1 20例,N2 19例,N3 2例;Ⅰ期13例,Ⅱ期13例,Ⅲ期19例,Ⅳ期12例。27例鼻咽部慢性炎癥常規(guī)石蠟切片標(biāo)本由我院病理科同期鼻咽部活檢標(biāo)本中選取,由HE染色確診為鼻咽部黏膜慢性炎。其中男性16例,女性11例,作為對(duì)照組。所有標(biāo)本離體后立即用10%中性甲醛固定,常規(guī)石蠟包埋,石蠟切片2μm厚連續(xù)切片,采用Envision TM二步法檢測(cè)VEGF、EGFR的表達(dá),用SPSS13.0軟件對(duì)實(shí)驗(yàn)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)。 結(jié)果: 1.VEGF、EGFR在鼻咽癌組織中均有陽性表達(dá)。 2. VEGF在鼻咽癌中的表達(dá)與淋巴結(jié)轉(zhuǎn)移及臨床分期有顯著相關(guān)性(P0.05),但和患者的年齡、性別、病理類型無相關(guān)性(P0.05)。 3. EGFR的表達(dá)和鼻咽癌的頸部淋巴結(jié)轉(zhuǎn)移存在顯著相關(guān)性(P0.05),但和患者的年齡、性別、病理類型、臨床分期無相關(guān)性(P0.05)。 4. EGFR在鼻咽癌中的表達(dá)與VEGF呈正相關(guān)(P0.01) 結(jié)論: 1.VEGF及EGFR在鼻咽癌組織中均有廣泛的表達(dá)。 2.VEGF的表達(dá)和鼻咽癌的頸部淋巴結(jié)轉(zhuǎn)移與臨床分期存在顯著相關(guān)性,但和患者的年齡、性別、病理類型無相關(guān)性。 3.EGFR的表達(dá)和鼻咽癌的頸部淋巴結(jié)轉(zhuǎn)移存在顯著相關(guān)性,但和患者的年齡、性別、病理類型、臨床分期無相關(guān)性。 4. EGFR在鼻咽癌中的表達(dá)與VEGF呈正相關(guān),兩者在鼻咽癌致病中可能有共同或相關(guān)途徑存在。
[Abstract]:Background and objective: nasopharyngeal carcinoma is the most common malignant tumor in the head and neck. The World Health Organization (WHO) estimates that about 80% of nasopharyngeal carcinogenesis and vascular endothelial growth factor (VEGF) is one of the most active angiogenic factors known at present, which can promote vascular endothelial proliferation and increase vascular permeability. It provides the matrix condition for the growth and migration of vascular endothelial cells, and has an important role in inhibiting the apoptosis of tumor cells. EGFR (vascular endothelial growth factor receptor) has high affinity for VEGF. The abnormal expression of EGFR could induce the transformation of normal cells into tumor cells and increase the ability of tumor invasion and metastasis. In this study, the expression of VEGF and EGFR in 57 cases of nasopharyngeal carcinoma and 27 cases of chronic inflammation of nasopharynx were detected by immunohistochemistry, and the relationship between the expression of VEGF and EGFR and clinical data was studied. Materials and methods: from 2009 to 2010, 57 nasopharyngeal biopsy specimens were first diagnosed as nasopharyngeal carcinoma in our hospital. Among them, 44 cases were male, 13 cases were female, the age was 17-74 years old, 19 cases were undifferentiated type of non-keratinizing carcinoma, 30 cases were non-keratinizing carcinoma, and 8 cases were poorly differentiated squamous cell carcinoma.
