【摘要】：目的分析白內(nèi)障小切口超聲乳化聯(lián)合角膜緣松解術矯正角膜散光的療效。方法選取我院住院并行手術治療的單純性白內(nèi)障患者30例(38眼),根據(jù)手術方式分為兩組:A組行小切口白內(nèi)障超聲乳化聯(lián)合角膜緣松解術(15例,18眼),B組行小切口白內(nèi)障超聲乳化術(15例,20眼)。觀察兩組患者術前、術后1周、3周、2個月的最佳矯正視力(best corrected visual acuity,BCVA)、角膜散光的變化,同時觀察術后兩組患者散光矯正量和術后并發(fā)癥的情況。結果 A、B組患者術后1周、3周、2個月的BCVA與術前比較均顯著提高,差異均有顯著統(tǒng)計學意義(均為P0.01);術后隨著時間的延長,BCVA逐漸提高,三個時間點間比較,差異均有顯著統(tǒng)計學意義(均為P0.01);A組術后3周、2個月BCVA均明顯高于B組,差異均有統(tǒng)計學意義(均為P0.05)。A、B組患者術后1周、3周、2個月的角膜散光值與術前比較均顯著降低,差異均有顯著統(tǒng)計學意義(均為P0.01);術后隨著時間的延長,角膜散光值逐漸降低,三個時間點間比較,差異均有顯著統(tǒng)計學意義(均為P0.01);A組術后1周、3周、2個月的角膜散光值明顯低于B組,差異均有顯著統(tǒng)計學意義(均為P0.01)。A組術后散光矯正量為(1.96±0.12)D,明顯高于B組的(1.19±0.09)D,差異有顯著統(tǒng)計學意義(P0.01)。A組和B組術后分別有1眼和2眼出現(xiàn)明顯的角膜霧狀混濁,術后3~4 d消失;A組和B組各有1眼出現(xiàn)眼部異物感,術后4~5 d消失。結論白內(nèi)障小切口超聲乳化聯(lián)合角膜緣松解術可以明顯減少術后患者的角膜散光,改善術后視力,安全性高。
[Abstract]:Objective to analyze the effect of small incision phacoemulsification combined with limbal lysis on corneal astigmatism. Methods Thirty patients (38 eyes) with simple cataract were divided into two groups: small incision phacoemulsification combined with corneal limbal lysis (15 cases, 18 eyes). Phacoemulsification of cataract (15 cases, 20 eyes). The best corrected visual acuity (best corrected visual acuity BCVA) and corneal astigmatism were observed before operation, 1 week and 2 months after operation, and the amount of astigmatism correction and postoperative complications were also observed. Results in group A, BCVA was significantly increased 1 week after operation and 2 months after operation (all P 0.01), BCVA was gradually increased with the extension of time, and the three time points were compared with each other. The BCVA in group A was significantly higher than that in group B at 3 weeks and 2 months after operation (all P0.05). The corneal astigmatism in group A was significantly lower than that in group B at 2 months. The corneal astigmatism value decreased gradually with the prolongation of time after operation, and the three time points were compared with each other. The corneal astigmatism in group A was significantly lower than that in group B at 1 week and 3 weeks after operation (all P0.01), and the value of corneal astigmatism in group A was significantly lower than that in group B at 2 months. The postoperative astigmatism correction in group A was (1.96 鹵0.12) D, which was significantly higher than that in group B (1.19 鹵0.09) D.The difference was statistically significant (P0.01) .There was a significant difference between group A (P0.01), group A (1 eye) and group B (2 eyes). One eye in group A and one in group B had ocular foreign body sensation on the 3rd and 4th day after operation, and disappeared on the 4th and 5th day after operation. Conclusion small incision phacoemulsification combined with limbal release can significantly reduce corneal astigmatism and improve postoperative visual acuity.
