【摘要】：目的：通過檢測角膜、虹膜及房水中抗體濃度的變化,探討體液免疫應(yīng)答與白內(nèi)障術(shù)后前房炎癥反應(yīng)的關(guān)系,并進(jìn)一步探討α晶狀體蛋白抗體與白內(nèi)障術(shù)后前房炎癥反應(yīng)的相關(guān)性。 方法：家兔40只,隨機(jī)選取8只,作為對照組(摘除正常眼球),其余家兔行超聲乳化術(shù),術(shù)后通過分層抽樣分為A組(術(shù)后1d摘除眼球)、B組(術(shù)后1周摘除眼球)、C組(術(shù)后2周摘除眼球)及D組(術(shù)后1個(gè)月摘除眼球),每組8只,并分別于術(shù)前、術(shù)后1d、1周、2周、1個(gè)月通過裂隙燈觀察房水閃輝及角膜水腫情況,免疫熒光法檢測角膜及虹膜中IgG抗體平均積分光密度值(MOD)。健康家兔40只,均行超聲乳化術(shù),分別于術(shù)前、術(shù)后1天、1周、2周、1個(gè)月抽取家兔房水樣品,通過間接酶聯(lián)免疫吸附試驗(yàn)(Indirect enzyme-linked immunosorbent assay,Indirect ELISA)檢測房水中α晶狀體蛋白抗體,用免疫比濁法檢測房水中IgG抗體含量。 結(jié)果：①平均積分光密度(MOD)：對照組：角膜：388.00±11.90,虹膜：1.04±0.23；A組：角膜：470.60±105.07,虹膜：177.70±61.66；B組：角膜：402.40±19.59,虹膜：42.41±3.42；C組：410.70±19.70,虹膜：5.15±0.61；D組：角膜：396.02±18.425,虹膜：1.34±0.37。各實(shí)驗(yàn)組虹膜中IgG積分光密度均數(shù)均較對照組升高,但D組與對照組統(tǒng)計(jì)分析結(jié)果無顯著差異性；各組家兔角膜中抗體與對照組均無統(tǒng)計(jì)學(xué)差異,但白內(nèi)障術(shù)后角膜中抗體MOD值的均數(shù)均較對照組升高,尤其是在白內(nèi)障術(shù)后早期重度水腫的家兔角膜中抗體的免疫熒光定量分析結(jié)果顯著升高。②房水中IgG及α晶狀體蛋白抗體檢測：房水中IgG抗體(mg/dl):術(shù)前：3.46±1.04,術(shù)后1天：8.24±2.56,術(shù)后1周：4.68±1.32,術(shù)后2周：4.22±1.08,術(shù)后1月：3.68±1.02。白內(nèi)障術(shù)后房水中IgG抗體含量增高,至一個(gè)月時(shí)接近正常水平。α晶狀體蛋白抗體檢測：白內(nèi)障術(shù)后1d,6只家兔房水中檢測到α晶狀體蛋白抗體,均角膜重度水腫,房水閃輝顯著。術(shù)后1周僅1只家兔房水閃輝仍較重,a晶狀體蛋白抗體陽性,且α晶狀體蛋白抗體與房水中IgG抗體含量有相關(guān)性。 結(jié)論;白內(nèi)障術(shù)后,體液免疫應(yīng)答與房水閃輝及角膜水腫有關(guān)；針對α晶狀體蛋白的體液免疫應(yīng)答只在白內(nèi)障術(shù)后重度前房炎癥反應(yīng)中發(fā)揮作用。
[Abstract]:Objective: to investigate the relationship between humoral immune response and anterior chamber inflammation after cataract surgery by detecting the concentration of antibodies in cornea, iris and aqueous humor. Furthermore, the relationship between 偽 -crystallin antibody and anterior chamber inflammation after cataract surgery was investigated. Methods: 40 rabbits were randomly selected as control group. The patients were divided into two groups by stratified sampling: group A (1 day after operation) and group C (removing eyeball at 1 week) and group D (1 month after operation with 8 eyes in each group). The glare of aqueous humor and corneal edema were observed by slit lamp and the mean integral optical density of IgG antibody in cornea and iris was detected by immunofluorescence. 40 healthy rabbits were treated with phacoemulsification. The samples of aqueous humor were collected from rabbits before operation, 1 day after operation and 1 week after operation. The antibodies of 偽 -crystallin in aqueous humor were detected by indirect enzyme-linked immunosorbent assayay assay (Elisa). The content of IgG antibody in aqueous humor was detected by immunoturbidimetric method. Results: the average integral optical density (MMD) of 1: 1: control group: cornea: 1: 388.00 鹵11.90, iris: 1. 04 鹵0. 23A: group A: cornea: 470.60 鹵105.07; iris: group B: 1: 402.40 鹵19. 59; iris: 42.41 鹵3. 422: C group: 410.70 鹵19.70; iris: 5. 15 鹵0. 61: d group: cornea: 396.02 鹵18.425; iris: 1. 34 鹵0. 377. The mean value of integrated optical density of IgG in the iris of each experimental group was higher than that of the control group, but there was no significant difference between group D and control group, and there was no significant difference in antibody in cornea between group D and control group. However, the mean value of MOD in cornea after cataract surgery was higher than that in control group. Especially in rabbits with severe edema after cataract surgery, the results of immunofluorescence quantitative analysis of antibodies in cornea increased significantly. 2 the detection of IgG and 偽 -crystallin antibodies in aqueous humor: IgG antibody in aqueous humor was measured by mg / dll: before operation: 3. 46 鹵1. 04, 1: 1 after operation. Day: 8.24 鹵2.56, 1 week: 1: 4.68 鹵1.32, 2 weeks after operation: 4. 22 鹵1. 08, 1 month after operation: 3. 68 鹵1. 02. The level of IgG antibody in aqueous humor after cataract increased to a normal level at one month. 偽 -crystallin antibody was detected in aqueous humor of 6 rabbits 1 day after cataract operation, all of them had severe corneal edema, and aqueous humor flashes were remarkable. 1 week after operation, only 1 rabbit aqueous humor was still positive for severe 偽 -crystallin antibody, and there was a correlation between 偽 -crystallin antibody and IgG antibody content in aqueous humor. Conclusion: the humoral immune response is related to aqueous flash and corneal edema after cataract, and the humoral immune response to 偽 -crystallin only plays a role in severe anterior chamber inflammation after cataract surgery.
