【摘要】：目的:探討慢性中耳炎細菌生物膜形成與細菌培養(yǎng)之間的相關(guān)性。方法:對32例慢性化膿性中耳炎及中耳膽脂瘤患者術(shù)中取得的樣本進行掃描電鏡檢查,并對中耳分泌物行細菌培養(yǎng);分析慢性中耳炎細菌生物膜形成與細菌培養(yǎng)結(jié)果之間的關(guān)系。結(jié)果:慢性化膿性中耳炎(活動期)與中耳膽脂瘤細菌生物膜形成率分別為87.5%、81.3%(P0.05)。慢性中耳炎掃描電鏡細菌生物膜結(jié)果與細菌培養(yǎng)結(jié)果之間比較,靈敏度為70.37%,特異度為60.00%,誤診率為40.00%,漏診率為29.63%,陽性預(yù)測值為90.46%,陰性預(yù)測值為27.27%,正確率為68.75%,約登指數(shù)為30.37%,Pearson相關(guān)系數(shù)為0.232(P0.05)。結(jié)論:慢性化膿性中耳炎(活動期)與中耳膽脂瘤均有較高的生物膜形成率,但中耳分泌物常規(guī)細菌培養(yǎng)結(jié)果不能反應(yīng)慢性中耳炎患者細菌生物膜的形成情況,需要探索更為可靠的細菌學(xué)實驗方法來準確反映慢性中耳炎的感染情況。
[Abstract]:Objective: to investigate the relationship between bacterial biofilm formation and bacterial culture in chronic otitis media. Methods: the samples obtained from 32 patients with chronic suppurative otitis media and cholesteatoma were examined by scanning electron microscope, and the secretion of middle ear was cultured. To analyze the relationship between bacterial biofilm formation and bacterial culture results in chronic otitis media. Results: the biofilm formation rates of chronic suppurative otitis media (active stage) and middle ear cholesteatoma were 87.5 and 81.3% respectively. The results of bacterial biofilm and bacterial culture in chronic otitis media were compared with those in scanning electron microscope. The sensitivity was 70.37 and the specificity was 60.00. the misdiagnosis rate was 40.00. the missed diagnosis rate was 29.63. The positive predictive value was 90.46 and the negative predictive value was 27.27. The accuracy was 68.75. The Pearson correlation coefficient was 0.232P 0.05. Conclusion: both chronic suppurative otitis media (active stage) and middle ear cholesteatoma have high biofilm formation rate, but the bacterial biofilm formation in patients with chronic otitis media can not be reflected by the results of routine bacterial culture of middle ear secretion. A more reliable bacteriological method is needed to accurately reflect the infection of chronic otitis media.
