【摘要】：目的:通過(guò)手術(shù)切除肥大的腺樣體和/或扁桃體治療由于腺樣體和/或扁桃體肥大而引起的兒童睡眠障礙性呼吸(SDB),探討手術(shù)治療對(duì)兒童SDB患者生活質(zhì)量的影響。 方法:對(duì)30例因扁桃體和/或腺樣體肥大的SDB患者,采用兒童OSAHS疾病特異性生活質(zhì)量調(diào)查(OSA-18)法,在術(shù)前一周內(nèi)和術(shù)后6個(gè)月進(jìn)行問(wèn)卷式調(diào)查,通過(guò)手術(shù)治療前后患兒生活質(zhì)量的變化,評(píng)價(jià)手術(shù)治療對(duì)SDB患者生活質(zhì)量的影響。 結(jié)果:其中單行腺樣體切除6例,單行雙側(cè)扁桃體切除2例,行雙側(cè)扁桃體切除+腺樣體切除22例;SDB患兒術(shù)后生活質(zhì)量總評(píng)及各維度評(píng)分較術(shù)前明顯下降(P均0.01),睡眠障礙、身體癥狀及對(duì)監(jiān)護(hù)人的影響這三個(gè)方面均得到了顯著改善;手術(shù)前有43.33%的患兒生活質(zhì)量受到嚴(yán)重影響,手術(shù)后有63.33%的患兒生活質(zhì)量得到明顯改善。 結(jié)論:兒童SDB嚴(yán)重影響患兒的生活質(zhì)量,扁桃體和/或腺樣體切除術(shù)是有效的治療方法,OSA-18評(píng)分可作為兒童SDB臨床輔助診斷和療效觀察的評(píng)價(jià)指標(biāo)。
[Abstract]:Objective: To investigate the effects of surgical treatment on the quality of life of children with SDB by surgical resection of hypertrophic adenoids and / or tonsillar in children with adenoids and / or tonsillar hypertrophy (SDB).
Methods: 30 cases of SDB patients with tonsillar and / or adenoid hypertrophy were investigated with a questionnaire survey of OSAHS disease specific quality of life (OSA-18), a week before and 6 months after the operation. The effects of hand operation on the quality of life of SDB patients were evaluated before and after the operation.
Results: there were 6 cases of unilateral adenoidectomy, 2 cases of unilateral tonsillectomy and 22 cases of bilateral tonsillectomy and adenoidectomy. The total evaluation of quality of life after operation in SDB children was significantly lower than before operation (P 0.01), sleep disorders, physical symptoms and influence on guardianship, all of the three aspects were significantly improved; before operation 43.33% of the children were severely affected by the quality of life, and 63.33% of them had a marked improvement in their quality of life.
Conclusion: SDB in children seriously affects the quality of life of children. Tonsillectomy and / or adenoidectomy is an effective treatment. The OSA-18 score can be used as an evaluation index for the clinical diagnosis and observation of SDB in children.
