【摘要】：目的:探討檢測(cè)眼肌前庭誘發(fā)肌源性電位(oVEMP)和頸肌前庭誘發(fā)肌源性電位(cVEMP)對(duì)單側(cè)原發(fā)性良性陣發(fā)性位置性眩暈(BPPV)患者進(jìn)行可能發(fā)病部位的定位診斷價(jià)值。方法:對(duì)52例單側(cè)原發(fā)性BPPV患者(BPPV組)和38例正常人(對(duì)照組)分別進(jìn)行氣導(dǎo)短純音誘發(fā)的oVEMP和cVEMP檢測(cè),分析兩種反射的引出率、潛伏期、振幅等數(shù)據(jù)。結(jié)果:BPPV組患側(cè)oVEMP的引出率為46.15%,cVEMP的引出率為67.31%;其健側(cè)oVEMP的引出率為48.08%,cVEMP的引出率為65.38%。對(duì)照組左側(cè)oVEMP的引出率84.21%,cVEMP的引出率92.11%;右側(cè)oVEMP的引出率為81.58%,cVEMP的引出率為94.74%。對(duì)照組雙側(cè)cVEMP和oVEMP的P1、N1潛伏期及N1-P1振幅值差異均無(wú)統(tǒng)計(jì)學(xué)意義。BPPV組和對(duì)照組cVEMP和oVEMP耳間振幅比及不對(duì)稱(chēng)率差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:單側(cè)原發(fā)性BPPV患者雙側(cè)前庭耳石器傳導(dǎo)通路功能受損狀況,可以通過(guò)oVEMP和cVEMP檢測(cè)進(jìn)行客觀評(píng)估,并且oVEMP的異常率比cVEMP高。
[Abstract]:Objective: to investigate the diagnostic value of detecting the oculomuscular vestibular evoked myogenic potential (OVEMPP) and the cervical vestibular evoked myogenic potential (CVEMPP) in patients with unilateral benign paroxysmal positional vertigo (BPPV). Methods: oVEMP and cVEMP were detected in 52 patients with unilateral primary BPPV and 38 normal subjects (control group). The extraction rate, latency and amplitude of the two reflexes were analyzed. Results the extraction rate of oVEMP in the affected side was 46.15 and that of camp in the control group was 67.31, and that of oVEMP in the healthy side was 48.08 and 65.38 respectively. In the control group, 84.21% of left oVEMP and 92.11% of cVEMP were extracted, while the rate of oVEMP in right side was 94.74%, 81.58% and 94.74%, respectively. There was no significant difference in P1N 1 latency and N1-P1 amplitude of bilateral cVEMP and oVEMP in control group. There were significant differences in amplitude ratio and asymmetry rate between cVEMP and oVEMP in BPPV group and control group (P 0.05). Conclusion: the functional impairment of bilateral vestibular otochoid conduction pathway in patients with unilateral primary BPPV can be evaluated objectively by oVEMP and cVEMP, and the abnormal rate of oVEMP is higher than that of cVEMP.
