融合分離手術(shù)對(duì)中年近視患者視覺質(zhì)量的影響

【摘要】: 目的:從視覺質(zhì)量的角度評(píng)價(jià)融合分離手術(shù)治療中年近視患者的療效。 方法:選取年齡35~45歲的近視患者100例,等效球鏡為-2.00D~-12.00D,術(shù)前最佳矯正視力≥1.0。隨機(jī)分為兩組:50例施行常規(guī)的LASIK為對(duì)照組(A組);50例施行融合分離手術(shù)(在LASIK中應(yīng)用monovision方式;即主導(dǎo)眼全矯正視遠(yuǎn),非主導(dǎo)眼低矯視近)為治療組(B組)。分別測(cè)量兩組患者手術(shù)前后單眼調(diào)節(jié)幅度的變化,測(cè)量術(shù)前雙眼戴鏡及術(shù)后半個(gè)月、1個(gè)月、3個(gè)月雙眼裸眼遠(yuǎn)視力、近視力、立體視、對(duì)比敏感度,并在術(shù)后3個(gè)月對(duì)所有患者進(jìn)行主觀視覺質(zhì)量及滿意度問卷調(diào)查。行兩組間及組內(nèi)的比較及分析。 結(jié)果:(1)兩組間術(shù)后各時(shí)間點(diǎn)雙眼遠(yuǎn)視力無統(tǒng)計(jì)學(xué)差異(P0.05);兩組間術(shù)后各時(shí)間點(diǎn)雙眼近視力有統(tǒng)計(jì)學(xué)差異(P0.05)。(2)兩組間各時(shí)間點(diǎn)雙眼對(duì)比敏感度無統(tǒng)計(jì)學(xué)差異(P0.05),術(shù)后半個(gè)月各頻率段兩組均有下降,至術(shù)后1~3個(gè)月逐漸恢復(fù),達(dá)到或超過術(shù)前水平。(3)兩組間術(shù)后各時(shí)間點(diǎn)立體視無統(tǒng)計(jì)學(xué)差異(P0.05),術(shù)后半個(gè)月兩組立體視均有下降趨勢(shì),術(shù)后1~3個(gè)月逐漸恢復(fù),但仍低于術(shù)前。(4)B組術(shù)后半個(gè)月主導(dǎo)眼及非主導(dǎo)眼調(diào)節(jié)幅度較術(shù)前有明顯的下降(P0.05),此后逐漸恢復(fù),至術(shù)后1~3個(gè)月時(shí)仍低于術(shù)前(P0.05)。(5)所有患者術(shù)后均無視物變形及色覺改變或失真。術(shù)后3個(gè)月A組有10例患者(22.7%)需戴閱讀眼鏡,而B組患者均不需戴閱讀眼鏡;術(shù)后3個(gè)月患者視遠(yuǎn)滿意度兩組無統(tǒng)計(jì)學(xué)差異(P0.05);視近滿意度兩組有統(tǒng)計(jì)學(xué)差異(P0.05)。 結(jié)論:融合分離手術(shù)治療中年患者近視療效明顯,優(yōu)于常規(guī)LASIK手術(shù),進(jìn)一步提高了患者的視覺質(zhì)量。
[Abstract]:Objective: to evaluate the efficacy of fusion separation surgery in the treatment of middle-aged myopia from the point of view of visual quality. Methods: 100 myopia patients aged 35 ~ 45 years were selected. The equivalent spherical lens was -2.00 DX -12.00 D.The preoperative best corrected visual acuity (BCVA) was 1.0 or better. Two groups of 50 cases were randomly divided into two groups: control group (n = 50) and control group (group A, n = 50) were treated with fusion and separation surgery (monovision mode was used in LASIK; that is, total correction of far distance in dominant eye, low corrected vision of non-dominant eye) was performed in group B as treatment group. The changes of the amplitude of monocular adjustment before and after operation were measured in both groups. The distance visual acuity, near vision, stereopsis, contrast sensitivity were measured before and after operation, half a month, 1 month, 3 months after operation. The subjective visual quality and satisfaction were investigated 3 months after operation. Comparison and analysis were made between the two groups and within groups. Results: (1) there was no significant difference in distant visual acuity between the two groups at each time point after operation (P 0.05); there was a significant difference in the near visual acuity between the two groups (P < 0.05); there was no significant difference in the contrast sensitivity between the two groups at different time points (P 0.05); there was no significant difference in the contrast sensitivity between the two groups (P 0.05), but there was no significant difference between the two groups (P < 0.05). The frequency of each month decreased in both groups. By 1 ~ 3 months after operation, there was no significant difference in stereopsis between the two groups (P < 0.05). The stereopsis of the two groups had a downward trend at half a month after operation, and gradually recovered 1 ~ 3 months after operation. But the adjustment amplitude of dominant eye and non-dominant eye in group B was significantly lower than that in group B (P 0.05), and then recovered gradually, and was still lower than that in group B 1 ~ 3 months after operation (P 0.05) all the patients ignored the distortion of objects and the change or distortion of color perception after operation. Three months after operation, 10 patients in group A need to wear reading glasses, while none in group B need to wear reading glasses, but there is no significant difference in distant satisfaction between the two groups at 3 months after operation (P 0.05), and there is a statistical difference between the two groups in the degree of near vision satisfaction (P 0.05). Conclusion: fusion separation surgery is more effective than conventional LASIK in the treatment of myopia in middle aged patients, and further improves the visual quality of the patients.
