【摘要】：目的:探討外鼻骨折的類(lèi)型及分布,探究外鼻骨折對(duì)鼻中隔的影響。方法:隨機(jī)選取因鼻外傷行鼻骨CT檢查并最終確診為外鼻骨折的患者60例,于計(jì)算機(jī)PACS系統(tǒng)上對(duì)該組患者的CT影像學(xué)資料進(jìn)行觀察及測(cè)量。結(jié)果:60例鼻外傷患者中共90側(cè)出現(xiàn)外鼻骨折,其中鼻骨骨折58側(cè)(64.4%),上頜骨額突骨折16側(cè)(17.8%),二者同時(shí)骨折16側(cè)(17.8%)。60例患者中,30例為單側(cè)外鼻骨折,其中伴外傷性鼻中隔偏曲14例,占46.7%;30例為雙側(cè)外鼻骨折,伴外傷性鼻中隔偏曲26例,占86.7%;二者差異有統(tǒng)計(jì)學(xué)意義(P0.01)。骨折僅累及鼻骨或上頜骨額突者24例,伴外傷性鼻中隔偏曲11例,占45.8%;骨折同時(shí)累及鼻骨和上頜骨額突者36例,伴外傷性鼻中隔偏曲29例,占80.6%;二者差異有統(tǒng)計(jì)學(xué)意義(P0.01)。外鼻骨折分型:Ⅰ型(單側(cè)外鼻骨折)16例,其中Ⅰa型(鼻骨或上頜骨額突骨折)15例(25.0%),Ⅰb型(鼻骨及上頜骨額突均骨折)1例(1.7%);Ⅱ型(雙側(cè)外鼻骨折)4例,其中Ⅱa型(鼻骨或上頜骨額突骨折)2例(3.3%),Ⅱb型(鼻骨及上頜骨額突均骨折)2例(3.3%);Ⅲ型(伴外傷性鼻中隔偏曲)40例(66.7%)。結(jié)論:外鼻骨折患者中伴有外傷性鼻中隔偏曲的比例較高,對(duì)癥狀較重或?qū)ν庑斡休^高需求的患者,應(yīng)于骨折1個(gè)月內(nèi)同時(shí)行鼻內(nèi)鏡下外鼻整復(fù)術(shù)及鼻中隔偏曲矯正術(shù),可以得到較滿(mǎn)意的效果。
[Abstract]:Objective: to investigate the types and distribution of external nasal fractures and the influence of external nasal fractures on nasal septum. Methods: 60 patients with nasal fracture were randomly selected for nasal bone CT examination and finally diagnosed as external nasal fracture. The CT imaging data of the patients were observed and measured on the computer PACS system. Results there were 90 cases of external nasal fracture in 60 patients with nasal trauma, including 58 cases of nasal bone fracture, 64.4 sides of nasal fracture, 16 sides of maxillary frontal process fracture, and 16 sides of bilateral fracture of maxillary frontal process. Among the 60 cases, 30 cases were unilateral external nasal fracture, among which 14 cases were accompanied by traumatic deviation of nasal septum. There were 30 cases of bilateral external nasal fracture and 26 cases of traumatic deviation of nasal septum (86.7%), the difference was statistically significant (P 0.01). There were only 24 cases involved in frontal process of nasal bone or maxilla, 11 cases with traumatic deviation of nasal septum, 36 cases with frontal process of nasal bone and maxillary bone, 29 cases with traumatic deviation of nasal septum (80.6%), the difference was statistically significant (P 0.01).
