【摘要】：目的探討微創(chuàng)玻璃體切割聯(lián)合超聲乳化術(shù)治療增生性糖尿病視網(wǎng)膜病變(proliferative diabetic retinopathy,PDR)的療效。方法選擇2011年1月至2012年12月我院收治的PDR患者120例(142眼),均給予玻璃體切割聯(lián)合超聲乳化吸出人工晶狀體植入術(shù)治療,根據(jù)手術(shù)切口不同隨機(jī)分為微創(chuàng)切口的觀察組(60例70眼)和常規(guī)切口的對(duì)照組(60例72眼)。術(shù)中觀察并記錄兩組患者的生命體征,術(shù)后觀察兩組患者的并發(fā)癥情況。術(shù)前和術(shù)后1周、2周、3周和4周檢測(cè)兩組患者的視力和角膜散光情況。結(jié)果兩組術(shù)中收縮壓、舒張壓及心率比較,差異均無統(tǒng)計(jì)學(xué)意義(均為P0.05);觀察組術(shù)后角膜水腫(8.33%)、角膜混濁(3.33%)、虹膜粘連(1.67%)發(fā)生率均明顯低于對(duì)照組(13.33%、6.67%、3.33%),差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05)。觀察組、對(duì)照組術(shù)后1周、2周、3周和4周視力較術(shù)前顯著增加,角膜散光較術(shù)前顯著降低,差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05);觀察組術(shù)后1周、2周、3周和4周視力高于對(duì)照組,角膜散光低于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05)。結(jié)論微創(chuàng)玻璃體切割聯(lián)合超聲乳化術(shù)治療PDR能夠促進(jìn)視力恢復(fù),減少并發(fā)癥發(fā)生。
[Abstract]:Objective to evaluate the efficacy of minimally invasive vitrectomy combined with phacoemulsification in the treatment of proliferative diabetic retinopathy (PDR) in patients with proliferative diabetic retinopathy. Methods from January 2011 to December 2012, 120 patients with PDR were treated with vitrectomy combined with phacoemulsification and intraocular lens implantation. According to the surgical incision, the patients were randomly divided into observation group (n = 60, n = 70) and control group (n = 60, n = 72). The vital signs of both groups were observed and recorded during operation and the complications were observed after operation. The visual acuity and corneal astigmatism were measured before and 1 week, 2 weeks, 3 weeks and 4 weeks after operation. Results there was no significant difference in systolic blood pressure, diastolic blood pressure and heart rate between the two groups (all P 0.05). The incidence of corneal edema, corneal opacification and iris adhesion in the observation group was significantly lower than that in the control group (P < 0.05). In the observation group, the visual acuity and corneal astigmatism in the control group increased significantly 1 week and 2 weeks after operation, and the corneal astigmatism decreased significantly (all P 0.05), and the visual acuity of the observation group was higher than that of the control group at 1 week, 2 weeks, 3 weeks and 4 weeks after operation, and the visual acuity of the control group was significantly higher than that of the control group. Corneal astigmatism was significantly lower than that in control group (P 0.05). Conclusion minimally invasive vitrectomy combined with phacoemulsification in the treatment of PDR can promote visual recovery and reduce complications.
