用聚合酶鏈反應(yīng)(PCR)法病因診斷非典型臨床表現(xiàn)的急性結(jié)膜炎研究

【摘要】:目的:通過(guò)對(duì)非典型急性結(jié)膜炎患者結(jié)膜囊分泌物進(jìn)行DNA提取以及PCR擴(kuò)增,探討聚合酶鏈反應(yīng)法在非典型臨床表現(xiàn)的急性結(jié)膜炎病因診斷中的臨床意義。 方法:選擇典型的急性細(xì)菌性結(jié)膜炎患者22人(22眼),典型急性病毒性結(jié)膜炎患者22人(22眼),典型過(guò)敏性結(jié)膜炎患者23人(23眼),非典型急性結(jié)膜炎患者44人(44眼),正常人22人(22眼),兩眼共患結(jié)膜炎者取較重的一側(cè)。所有受試者結(jié)膜囊分泌物提取的DNA分成兩部分,分別加入細(xì)菌引物及腺病毒引物進(jìn)行兩次PCR擴(kuò)增,凝膠電泳后在凝膠成像分析系統(tǒng)中觀察結(jié)果。對(duì)于非典型急性結(jié)膜炎組患者同時(shí)進(jìn)行細(xì)菌培養(yǎng)觀察結(jié)果。 結(jié)果:1.典型急性細(xì)菌性結(jié)膜炎組(Ⅰ)有9眼擴(kuò)增出520bp大小條帶,陽(yáng)性率40.9%;2眼擴(kuò)增出1004bp條帶,陽(yáng)性率9.0%;520bp兼1004bp條帶有1眼,陽(yáng)性率4.54%。典型急性病毒性結(jié)膜炎組(Ⅱ)有2眼擴(kuò)增出520bp大小條帶,陽(yáng)性率9.0%;8眼擴(kuò)增出1004bp條帶,陽(yáng)性率36.3%;520bp兼1004bp條帶有1眼,陽(yáng)性率4.54%。非典型急性結(jié)膜炎組(Ⅲ)有12眼擴(kuò)增出520bp大小條帶,陽(yáng)性率27.27%;11眼擴(kuò)增出1004bp大小條帶,陽(yáng)性率25%;520bp兼1004bp條帶有3眼陽(yáng)性率6.81%。以上兩組間相互比較結(jié)果只有(Ⅰ)組和(Ⅱ)組有顯著性差異(X2=5.88、4.66,p0.05)。(Ⅰ)組和(Ⅲ)組(X2=2.77、2.29、0.124、p0.05)、(Ⅱ)組和(Ⅲ)組(X2=2.97、1.76、0.124,p0.05)均無(wú)顯著性差異。2.非典型急性結(jié)膜炎組(Ⅲ)細(xì)菌培養(yǎng)法有17眼培養(yǎng)出陽(yáng)性,陽(yáng)性率為38.6%與PCR檢測(cè)法比較無(wú)顯著性差異(X2=1.77,P0.05)。 結(jié)論:1.PCR檢測(cè)法對(duì)非典型的急性結(jié)膜炎進(jìn)行病因診斷具有臨床應(yīng)用意義。 2.雖然PCR檢測(cè)法陽(yáng)性檢出率較細(xì)菌培養(yǎng)法無(wú)明顯差異,但對(duì)樣本要求不高,且經(jīng)濟(jì)又時(shí)間短,對(duì)急性結(jié)膜炎診斷具有良好的應(yīng)用前景,值得推廣。
[Abstract]:Objective: to investigate the clinical significance of polymerase chain reaction (PCR) in the etiological diagnosis of atypical acute conjunctivitis by DNA extraction and PCR amplification of conjunctival sac secretions from patients with atypical acute conjunctivitis. Methods: 22 eyes of 22 patients with typical acute bacterial conjunctivitis, 22 patients with acute viral conjunctivitis, 23 patients with typical allergic conjunctivitis, 44 patients with atypical acute conjunctivitis and 44 patients with atypical acute conjunctivitis were selected. 22 eyes of 22 normal people had conjunctivitis and the severe side of conjunctivitis was taken. The DNA extracted from conjunctival sac secretion was divided into two parts. Bacterial primer and adenovirus primer were added to amplify PCR twice. The results were observed in the gel imaging analysis system after gel electrophoresis. The results of bacterial culture were observed in patients with atypical acute conjunctivitis at the same time.
