鼻咽癌中HIF-2α、GLUT-1及OPN的表達(dá)及其意義

【摘要】: 目的研究乏氧誘導(dǎo)因子2α(HIF-2a)、葡萄糖轉(zhuǎn)運(yùn)蛋白1(GLUT-1)及骨橋蛋白(OPN)蛋白在鼻咽癌中的表達(dá)及其與鼻咽癌生物學(xué)行為及放療反應(yīng)性之間的關(guān)系,探討HIF-2a、GLUT-1及OPN在鼻咽癌診斷、臨床分期、治療及預(yù)后評估等方面的作用。 方法應(yīng)用免疫組織化學(xué)方法檢測51例初診鼻咽癌組織和20例鼻咽炎性組織中HIF-2a、GLUT-1及OPN蛋白的表達(dá),并對29例患者放療前、放療期間、放療結(jié)束時活檢組織中HIF-2α、GLUT-1及OPN的蛋白表達(dá)進(jìn)行動態(tài)檢測。應(yīng)用酶聯(lián)免疫吸附試驗(yàn)(ELISA)檢測30例鼻咽癌患者及20例正常人的血清OPN水平。 結(jié)果鼻咽癌組織中HIF-2α、GLUT-1及OPN蛋白的陽性表達(dá)率分別為62.7%、49%和52.9%,明顯高于鼻咽慢性炎性組織的20%、10%和10%(P0.05)。HIF-2a蛋白的表達(dá)與T分期、是否伴頸部淋巴結(jié)轉(zhuǎn)移及臨床分期相關(guān)(P0.05)。GLUT-1蛋白的表達(dá)與臨床分期、是否伴有頸部淋巴結(jié)轉(zhuǎn)移顯著相關(guān)(P0.05),而兩者的表達(dá)與患者性別、年齡無關(guān)(P0.05)。OPN蛋白的表達(dá)與患者的性別、年齡、T分期、是否伴頸部淋巴結(jié)轉(zhuǎn)移及臨床分期均無明顯相關(guān)性(P0.05)。鼻咽癌組織中HIF-2α和GLUT-1蛋白的表達(dá)呈正相關(guān)(P0.01),OPN蛋白表達(dá)與HIF-2a和GLUT-1蛋白表達(dá)間未見相關(guān)性。29例動態(tài)觀察的患者中,放療期間HIF-2α、GLUT-1及OPN蛋白的表達(dá)強(qiáng)度較放療前明顯降低(P0.05)。鼻咽癌組血清OPN水平161.02±44.80ng/ml明顯高于正常對照組58.89±24.65ng/ml,兩者的差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。血清OPN水平與臨床分期、是否伴有頸部淋巴結(jié)轉(zhuǎn)移相關(guān)(P0.05),而與患者性別、年齡、T分期無明顯相關(guān)(P0.05)。鼻咽癌血清OPN水平與其腫瘤組織中HIF-2α、GLUT-1和OPN蛋白的表達(dá)之間未見相關(guān)性。 結(jié)論HIF-2α、GLUT-1和OPN蛋白的高表達(dá)在鼻咽癌的發(fā)生、發(fā)展過程中有重要作用。共同檢測HIF-2a與GLUT-1的表達(dá)對于鼻咽癌的臨床分期、判斷療效及預(yù)后評估有重要意義。血清OPN水平檢測可作為鼻咽癌的診斷、分期及預(yù)后判斷的輔助指標(biāo)。
[Abstract]:Objective to study the expression of hypoxia inducible factor 2 偽 (HIF-2a), glucose transporter (GLUT-1) and osteopontin (OPN) protein in nasopharyngeal carcinoma (NPC) and its relationship with the biological behavior and radiotherapy reactivity of nasopharyngeal carcinoma (NPC), and to explore the role of HIF-2aGLUT-1 and OPN in the diagnosis and clinical staging of nasopharyngeal carcinoma (NPC). The role of treatment and prognosis evaluation. Methods Immunohistochemical method was used to detect the expression of HIF-2aA1GLUT-1 and OPN protein in 51 nasopharyngeal carcinoma tissues and 20 nasopharyngitis tissues. The protein expression of HIF-2 偽 -GLUT-1 and OPN in biopsy tissue was detected dynamically at the end of radiotherapy. The serum OPN levels of 30 patients with nasopharyngeal carcinoma and 20 normal controls were detected by Elisa. Results the positive expression rates of HIF-2 偽 -GLUT-1 and OPN protein in nasopharyngeal carcinoma were 62.7% and 52.9%, respectively, which were significantly higher than those in chronic nasopharyngeal inflammatory tissue (20%), 10%(P0.05).HIF-2a protein expression and T stage. The expression of P0.05, GLUT-1 protein associated with cervical lymph node metastasis and clinical stage was significantly correlated with the clinical stage, and whether there was a significant correlation between the expression of P0.05 protein and the patient's sex, but the expression of both proteins was not related to the gender of the patient, nor was the expression of P0.05. OPN protein correlated with the sex of the patient. There was no significant correlation between age T stage, cervical lymph node metastasis and clinical stage (P 0.05). There was no positive correlation between the expression of HIF-2 偽 and GLUT-1 protein and the expression of HIF-2a and GLUT-1 in nasopharyngeal carcinoma. The expression intensity of HIF-2 偽 GLUT-1 and OPN during radiotherapy was significantly lower than that before radiotherapy. The serum OPN level in nasopharyngeal carcinoma group (161.02 鹵44.80ng/ml) was significantly higher than that in normal control group (58.89 鹵24.65 ng / ml), and the difference was statistically significant (P 0.05). Serum OPN level was correlated with clinical stage, cervical lymph node metastasis or not, but not with gender and age T stage. There was no correlation between the serum OPN level and the expression of HIF-2 偽 -GLUT-1 and OPN protein in nasopharyngeal carcinoma. Conclusion the overexpression of HIF-2 偽 -GLUT-1 and OPN protein plays an important role in the pathogenesis and development of nasopharyngeal carcinoma. The expression of HIF-2a and GLUT-1 is of great significance in clinical staging, evaluation of curative effect and prognosis of nasopharyngeal carcinoma. Serum OPN level can be used as an auxiliary index for diagnosis, staging and prognosis of nasopharyngeal carcinoma.
