消毒時(shí)機(jī)的選擇對(duì)內(nèi)眼手術(shù)患者術(shù)后眼內(nèi)感染的影響研究

【摘要】:目的探討不同消毒時(shí)機(jī)對(duì)內(nèi)眼手術(shù)患者術(shù)后眼內(nèi)感染的預(yù)防效果,為減少眼科患者術(shù)后感染、提高預(yù)后提供參考。方法選擇2011年6月-2013年6月眼科行白內(nèi)障手術(shù)的1 264例患者作為觀察對(duì)象,隨機(jī)分為3組:A組421例,術(shù)前生理鹽水沖洗+結(jié)膜囊沖洗+無(wú)痛碘結(jié)膜囊消毒,B組421例,術(shù)前生理鹽水沖洗+結(jié)膜囊沖洗+無(wú)痛碘結(jié)膜囊消毒,待貼好眼貼膜后再進(jìn)行生理鹽水沖洗+結(jié)膜囊沖洗+無(wú)痛碘結(jié)膜囊消毒,C組422例,術(shù)前生理鹽水沖洗+結(jié)膜囊沖洗+無(wú)痛碘結(jié)膜囊消毒+術(shù)后無(wú)痛碘結(jié)膜囊消毒;用無(wú)菌棉擦拭上下瞼彎隆結(jié)膜并迅速放入肉湯試管進(jìn)行結(jié)膜囊細(xì)菌培養(yǎng),采用SPSS13.0軟件對(duì)3組數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果 A、B、C 3組患者眼內(nèi)感染率分別為11.16%、8.31%、5.45%,差異有統(tǒng)計(jì)學(xué)意義(P0.05);共檢出105株病原菌,以金黃色葡萄球菌為主25株占23.81%,其次為枯草芽胞桿菌黑色變種芽胞19株占18.10%。結(jié)論術(shù)前生理鹽水沖洗+結(jié)膜囊沖洗+無(wú)痛碘結(jié)膜囊消毒+術(shù)后無(wú)痛碘結(jié)膜囊消毒是預(yù)防內(nèi)眼術(shù)后感染的有效措施,可以常規(guī)實(shí)施。
[Abstract]:Objective to explore the preventive effect of different disinfection time on postoperative intraocular infection in patients undergoing internal ophthalmic surgery, and to provide reference for reducing postoperative infection and improving prognosis of ophthalmic patients. Methods 1 264 patients undergoing cataract surgery from June 2011 to June 2013 were randomly divided into 3 groups: group A (n = 421) and group B (n = 421). The painless iodine conjunctival sac was irrigated with saline before operation, and 422 cases in group C were treated with saline to rinse the conjunctival sac after the application of eye patch, and then to rinse the conjunctival sac with normal saline for disinfecting painless iodide conjunctival sac. The painless iodide conjunctival sac was disinfected after disinfecting the painless iodine conjunctival sac with normal saline before operation, and the conjunctival sac was cleaned with aseptic cotton and quickly put into the broth test tube for bacterial culture of the conjunctival sac. Three groups of data were analyzed by SPSS13.0 software. Results the intraocular infection rate was 11.16% 8.31% and 5.45% (P < 0.05) respectively in group C, 105 strains of pathogenic bacteria were detected, 25 strains of Staphylococcus aureus accounted for 23.81%, followed by 19 strains of Bacillus subtilis var. nigra spores (18.10%). Conclusion the disinfection of painless iodide conjunctival sac after disinfection of iodide conjunctival sac with normal saline before operation is an effective measure to prevent intraocular and postoperative infection and can be carried out routinely.
