【摘要】： 目的 通過臨床病例隨訪研究,觀察72例鼻咽癌患者的生存情況,了解鼻咽癌組織中Survivin, Cyclin D1和nm23-H1蛋白表達(dá)與放療預(yù)后的關(guān)系。 方法 收集2000年1月至2004年5月間在我科初治、有完整臨床及病理資料的鼻咽癌72例,對(duì)患者治療前鼻咽部病理標(biāo)本進(jìn)行S—P法免疫組織化學(xué)技術(shù)檢測(cè)其中Survivin, Cyclin D1和nm23-H1蛋白表達(dá),并臨床隨訪5年以上結(jié)果 (1)72例鼻咽癌患者3年生存率為65.2%,5年生存率為44.4%。 (2) Survivin蛋白陽(yáng)性表達(dá)率Ⅰ期為75.0%(3/4),與放療預(yù)后無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅱ期為64.3%(18/28),有統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅲ期為33.3%(11/33),有統(tǒng)計(jì)學(xué)意義；Ⅳa期為71.4%(5/7),有統(tǒng)計(jì)學(xué)意義。 (3) Cyclin D1蛋白陽(yáng)性表達(dá)率Ⅰ期為50.0%(2/4),提示與放療預(yù)后無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅱ期的陽(yáng)性表達(dá)率為67.9%(19/28),提示有統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅲ期的陽(yáng)性率為48.5%(16/33),有統(tǒng)計(jì)學(xué)意義；Ⅳa期的陽(yáng)性率為71.4%(5/7),有統(tǒng)計(jì)學(xué)意義。 (4)Nm23-H1蛋白陽(yáng)性表達(dá)率Ⅰ期陽(yáng)性表達(dá)為100.0%(4/4),與放療預(yù)后無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅱ期為64.39%(18/28),提示有統(tǒng)計(jì)學(xué)意義(P0.05)；Ⅲ期為72.7%(24/33),有統(tǒng)計(jì)學(xué)意義；Ⅳa期的陽(yáng)性率為57.1%(4/7),有統(tǒng) 結(jié)論 鼻咽癌組織中Survivin、Cyclin D1蛋白表達(dá)與放療預(yù)后呈負(fù)相關(guān),nm23-H1蛋白的表達(dá)程度與放療預(yù)后呈正相關(guān)�？勺鳛楸茄拾┗颊哳A(yù)測(cè)放療預(yù)后的一個(gè)獨(dú)立因素。
[Abstract]:objective
To observe the survival of 72 patients with nasopharyngeal carcinoma and to understand the relationship between the expression of Survivin, Cyclin D1 and nm23-H1 protein and the prognosis of radiotherapy in nasopharyngeal carcinoma.
Method
72 cases of nasopharyngeal carcinoma with complete clinical and pathological data were collected from January 2000 to May 2004 with complete clinical and pathological data. The S P immunohistochemical technique was used to detect the expression of Survivin, Cyclin D1 and nm23-H1 protein in the pathological specimens of the nasopharynx before treatment, and the results were followed up for more than 5 years.
(1) the 3 year survival rate of 72 nasopharyngeal carcinoma patients was 65.2%, and the 5 year survival rate was 44.4%.
(2) the positive expression rate of Survivin protein was 75% (3/4), and was not statistically significant with the prognosis of radiotherapy (P0.05), and 64.3% (18/28) in stage II (P0.05), and 33.3% (11/33) in stage III, and 71.4% (5/7) in stage IV A, with statistical significance.
(3) the positive expression rate of Cyclin D1 protein was 50% (2/4), indicating no statistical significance with radiotherapy (P0.05), and the positive rate in stage II was 67.9% (19/28), indicating a statistical significance (P0.05), and the positive rate in stage III was 48.5% (16/33), and the positive rate in stage IV A was 71.4% (5/7), with statistical significance.
(4) the positive expression of Nm23-H1 protein positive expression was 100% (4/4), and was not statistically significant with the prognosis of radiotherapy (P0.05), and 64.39% (18/28) in stage II (P0.05), and 72.7% (24/33) in stage III; the positive rate in stage IV A was 57.1% (4/7), and there was a significant difference.
conclusion
The expression of Survivin, Cyclin D1 protein in nasopharyngeal carcinoma is negatively correlated with the prognosis of radiotherapy, and the expression of nm23-H1 protein is positively correlated with the prognosis of radiotherapy. It can be used as an independent prognostic factor for patients with nasopharyngeal carcinoma to predict the prognosis of radiotherapy.
