【摘要】：目的：探討喉咽鱗癌組織中細(xì)胞角蛋白4(CK4)的表達(dá)及其與腫瘤發(fā)生、侵襲和轉(zhuǎn)移的關(guān)系。 方法：應(yīng)用免疫組織化學(xué)方法檢測(cè)60例喉咽鱗癌患者的癌旁正常組織和癌組織標(biāo)本中CK4的表達(dá),應(yīng)用VIDAS圖像分析儀分析其圖像的積分光密度(iOD),應(yīng)用SPSS17.0進(jìn)行統(tǒng)計(jì)學(xué)處理。 結(jié)果：在癌旁正常組織中,CK4表達(dá)明顯、完整,而在鱗癌組織中,CK4的表達(dá)明顯下降,其表達(dá)差異有顯著性χ2=73.594, P0.05)。在喉咽鱗癌組織中CK4的表達(dá)隨病理分化程度下降而下降,完整性亦逐漸破壞甚至消失,其變化的差異有統(tǒng)計(jì)學(xué)意義(P0.05)；CK4在T1-T2期的表達(dá)最強(qiáng),在T3、T4期的表達(dá)依次減弱,其變化的差異有統(tǒng)計(jì)學(xué)意義(F=105.901,P0.05)；CK4在Ⅰ-Ⅱ期的表達(dá)最強(qiáng),在Ⅲ期、Ⅳ期中的表達(dá)逐漸減弱,其變化的差異有統(tǒng)計(jì)學(xué)意義(F=138.815,P0.05)；CK4在無淋巴結(jié)轉(zhuǎn)移的鱗癌組織中的表達(dá)較強(qiáng),而在有淋巴結(jié)轉(zhuǎn)移的鱗癌組織中表達(dá)較弱,其變化的差異有統(tǒng)計(jì)學(xué)意義(T=11.152,P0.05)；CK4表達(dá)陽性組較陰性組的綜合治療后5年生存率明顯提高,P0.05。 結(jié)論：伴隨著喉咽鱗癌病理學(xué)分化程度的降低CK4的表達(dá)逐漸降低；CK4在喉咽鱗癌組織中的低表達(dá)與腫瘤發(fā)生、侵襲和轉(zhuǎn)移等生物學(xué)特性密切相關(guān)；CK4在喉咽鱗癌組織中的低表達(dá)對(duì)探討喉咽鱗癌的生物學(xué)行為有一定的意義,并有望成為判斷喉咽鱗癌惡性程度的生物學(xué)標(biāo)記物。
[Abstract]:Objective: to investigate the expression of cytokeratin 4 CK4 in laryngeal squamous cell carcinoma (LSCC) and its relationship with carcinogenesis, invasion and metastasis. Methods: immunohistochemical method was used to detect the expression of CK4 in the normal tissues and tissues of 60 patients with larynx squamous cell carcinoma. The integral optical density (IOD) was analyzed by VIDAS image analyzer and SPSS17.0 was used for statistical analysis. Results: the expression of CK-4 in adjacent normal tissues was obvious and intact, while in squamous cell carcinomas, the expression of CK-4 was significantly decreased (蠂 ~ 2 / 73.594, P 0.05). In larynx squamous cell carcinoma, the expression of CK4 decreased with the decrease of pathological differentiation, and the integrality was gradually destroyed or even disappeared.
