HLA-DR7在增殖性糖尿病視網(wǎng)膜病變中表達(dá)的研究

【摘要】: 背景與目的:隨著生活方式和飲食結(jié)構(gòu)的變化,糖尿病(Diabetes Mellitus, DM)患病率逐漸升高,糖尿病視網(wǎng)膜病變(Diabetic Retinopathy, DR)作為糖尿病的最常見和最嚴(yán)重的并發(fā)癥之一也隨之升高。糖尿病視網(wǎng)膜病變?yōu)樗拇笾旅ぱ壑?也是五大死亡原因之一,嚴(yán)重影響患者的生活質(zhì)量與社會(huì)經(jīng)濟(jì)的發(fā)展。糖尿病視網(wǎng)膜病變的發(fā)病機(jī)制復(fù)雜,其確切的發(fā)病機(jī)制目前尚不清楚,但個(gè)體對(duì)糖尿病視網(wǎng)膜病變的易感性越來越受到關(guān)注,基因?qū)μ悄虿∫暰W(wǎng)膜病變發(fā)生發(fā)展的影響的研究已成為目前國(guó)內(nèi)外研究的熱點(diǎn)。 本研究旨在分析HLA-DR在2型糖尿病并發(fā)增殖性糖尿病視網(wǎng)膜病變(proliferative diabetic retinopathy, PDR)患者中的表達(dá)和HLA-DR7對(duì)糖尿病視網(wǎng)膜病病變發(fā)生發(fā)展的影響,并探討PDR的遺傳易感因素,為臨床PDR的治療和預(yù)防提供科學(xué)依據(jù),同時(shí)也為大量DM患者克服社會(huì)心理問題提供依據(jù)。 方法:隨機(jī)選取在山西醫(yī)科大學(xué)第一醫(yī)院眼科門診就診和住院治療的山西省漢族2型糖尿病病程大于10年未并發(fā)糖尿病視網(wǎng)膜病變的患者30例作為病例對(duì)照組,選取T2DM并發(fā)PDR的患者30例作為病例組,30例健康人做為正常對(duì)照組作為研究對(duì)象。抽取外周血4ml并提取DNA,對(duì)HLA-DR3、DR7、DR11基因進(jìn)行PCR擴(kuò)增及基因頻率的測(cè)定。診斷由臨床檢查確診包括眼底檢查和FFA檢查及OCT等檢查。對(duì)采用SPSS16.0軟件包對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,以p≤0.05為統(tǒng)計(jì)學(xué)有意義。 結(jié)果:HLA-DR7在病例對(duì)照組者中的平均值為88.53±8.31,在病例組中的平均值為18.88±0.82; HLA-DR3, DR11頻率在病例組中的平均值分別為80.55±3.55,84.15±6.51,在正常對(duì)照組中頻率平均值分別為14.48±0.182,27.86±1.415. HLA-DR3、DR7、DR11在NDM組,DM組和PDR各組中的表達(dá)比較均p0.001,差異有統(tǒng)計(jì)學(xué)意義。 結(jié)論:HLA-DR7在山西漢族T2DM病程大于10年未并發(fā)的DR的患者中表達(dá)增高,提示HLA-DR7對(duì)PDR的發(fā)生發(fā)展可能有保護(hù)作用;HLA-DR3在PDR中表達(dá)增高,提示PDR有明顯的遺傳易感傾向。
[Abstract]:Background & objective: with the change of life style and diet structure, the prevalence rate of diabetes mellitus Mellitus (DM) is increasing, and diabetic retinopathy (DR) is one of the most common and serious complications of diabetes mellitus. Diabetic retinopathy is one of the four blind eyes and one of the five leading causes of death, which seriously affects the quality of life and the development of social economy. The pathogenesis of diabetic retinopathy is complex, and its exact pathogenesis is still unclear, but the susceptibility of individual to diabetic retinopathy has been paid more and more attention. The effect of gene on the development of diabetic retinopathy has become a hot topic at home and abroad. The purpose of this study was to investigate the expression of HLA-DR in type 2 diabetic patients with proliferative diabetic retinopathy, PDR) and the effect of HLA-DR7 on the development of diabetic retinopathy, and to explore the genetic predisposing factors of PDR. To provide scientific basis for the treatment and prevention of clinical PDR, and also for a large number of DM patients to overcome psychosocial problems. Methods: thirty patients with type 2 diabetes mellitus in Shanxi Han nationality who had not complicated with diabetic retinopathy for more than 10 years were randomly selected as the control group. Thirty patients with T2DM complicated with PDR were selected as case group and 30 healthy persons as control group. 4ml was extracted from peripheral blood and DNA was extracted. HLA-DR3 DR7 DR11 gene was amplified by PCR and the gene frequency was determined.
