【摘要】：目的本文通過60例(60眼)視神經(jīng)挫傷住院患者的臨床資料,對(duì)鼠神經(jīng)生長(zhǎng)因子結(jié)合葛根素治療視神經(jīng)挫傷進(jìn)行臨床療效觀察;對(duì)其流行病學(xué)、診斷、治療方法等臨床資料進(jìn)行了總結(jié),并對(duì)治療效果進(jìn)行了比較和探討。 方法本課題對(duì)2008年5月-2009年11月在哈爾濱二四二醫(yī)院眼科、哈醫(yī)大一院眼科進(jìn)行治療的60例視神經(jīng)挫傷患者建立臨床觀察表,進(jìn)行臨床觀察研究,隨機(jī)將視神經(jīng)挫傷患者60例(60眼)分為2組,治療組30例(30眼),對(duì)照組30例(30眼)。治療組主要采用鼠神經(jīng)生長(zhǎng)因子30μg,用2 mL生理氯化鈉溶液或注射用水溶解,一日一次肌肉注射,平均治療21天,葛根素250mL一日一次靜脈注射,同時(shí)常規(guī)用藥治療。對(duì)照組除不加用鼠神經(jīng)生長(zhǎng)因子,葛根素外,其他用藥同治療組,通過視力、視覺誘發(fā)電位及視野對(duì)兩種治療方法在治療前后及兩組間進(jìn)行比較。 結(jié)果(1)鼠神經(jīng)生長(zhǎng)因子結(jié)合葛根素治療視神經(jīng)挫傷總有效率為86.7% ,對(duì)照組為63.3% ,兩組比較有顯著性差異具有統(tǒng)計(jì)學(xué)意義(χ2=3.675,P0.05)。(2)視覺誘發(fā)電位方面,通過治療前后及兩組間P100潛伏期及幅值進(jìn)行比較,兩組患者治療前對(duì)比無統(tǒng)計(jì)學(xué)意義(p0.05),兩組患者治療后比較有統(tǒng)計(jì)學(xué)意義(P0.05),治療組治療后優(yōu)于對(duì)照組治療后,并且兩組患者治療后優(yōu)于兩組患者治療前,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。(3)在視野方面,通過治療前后及兩組間視野平均光敏度及平均缺損度進(jìn)行比較,兩組患者治療前對(duì)比無統(tǒng)計(jì)學(xué)意義(p0.05),兩組患者治療后比較有統(tǒng)計(jì)學(xué)意義(P0.05),治療組治療后優(yōu)于對(duì)照組治療后,并且兩組患者治療后優(yōu)于兩組患者治療前,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。 結(jié)論1鼠神經(jīng)生長(zhǎng)因子結(jié)合葛根素對(duì)視神經(jīng)挫傷的治療具有肯定療效,值得推廣。2相對(duì)于視力檢查,VEP是客觀、準(zhǔn)確、臨床實(shí)用性很強(qiáng)的診察方法。3常規(guī)治療基礎(chǔ)上加用鼠神經(jīng)生長(zhǎng)因子結(jié)合葛根素對(duì)視神經(jīng)挫傷的療效優(yōu)于單純的常規(guī)激素治療;
[Abstract]:Objective to observe the clinical efficacy of nerve growth factor (NGF) combined with puerarin in the treatment of optic nerve contusion in 60 patients (60 eyes) with optic nerve contusion. The clinical data such as treatment methods were summarized, and the therapeutic effects were compared and discussed. Methods from May 2008 to November 2009, 60 patients with optic nerve contusion were treated in Harbin No.2 Hospital. 60 cases of optic nerve contusion were randomly divided into two groups: 30 eyes in the treatment group and 30 eyes in the control group. The treatment group was treated with nerve growth factor (NGF) 30 渭 g, dissolved with 2 mL physiological sodium chloride solution or injected with water, intramuscularly injected once a day for an average of 21 days, puerarin 250mL was injected intravenously once a day, and treated with routine medication. In the control group, the visual acuity, visual evoked potential and visual field were compared between the two groups before and after treatment, except for rat nerve growth factor and puerarin. Results 1) the total effective rate of nerve growth factor (NGF) combined with puerarin in the treatment of optic nerve contusion was 86.7%, and that in the control group was 63.3%. The latency and amplitude of P100 before and after treatment were compared between the two groups. There was no significant difference between the two groups before and after treatment, but there was significant difference between the two groups after treatment. The treatment group was better than the control group after treatment. After treatment, the difference between the two groups was statistically significant (P < 0.05). The visual field of the two groups was compared by comparing the average Guang Min degree and the average defect degree before and after treatment, and the difference between the two groups before and after treatment. There was no significant difference between the two groups before and after treatment, there was significant difference between the two groups after treatment, the treatment group was better than the control group after treatment, and the two groups were better than the two groups before treatment, the difference was statistically significant. Conclusion 1Rat nerve growth factor combined with puerarin has positive curative effect on optic nerve contusion, which is worth popularizing compared with visual acuity examination and VEP is objective and accurate. On the basis of routine treatment of rat nerve growth factor combined with puerarin, the effect of routine therapy on optic nerve contusion was better than that of routine hormone therapy.
