【摘要】： 目的：內(nèi)眥贅皮又稱為蒙古皺襞,在亞洲人群中發(fā)生率為50%左右,在單瞼人群中發(fā)生率超過70%。因此內(nèi)眥贅皮矯正術(shù)是一種常見的美容手術(shù)。內(nèi)眥贅皮的存在縮短了瞼裂的長(zhǎng)度,不符合面部的美學(xué)標(biāo)準(zhǔn),重度內(nèi)眥贅皮甚至遮擋了部分視野。很多手術(shù)醫(yī)生由于害怕造成明顯的內(nèi)眥瘢痕而不敢開展這項(xiàng)手術(shù)。我們采用Park法,內(nèi)眥韌帶縮短和重瞼術(shù)同期進(jìn)行。通過臨床驗(yàn)證,此方法對(duì)所有類型內(nèi)眥贅皮矯正效果滿意。 方法：自2007年到2009年收治內(nèi)眥贅皮患者125例,其中上瞼型48例,共96側(cè),瞼板型52例,共104側(cè),下瞼型25例,共50側(cè),年齡20-40歲,平均年齡為30歲。我們根據(jù)設(shè)計(jì)線切開皮膚,去除適量的皮膚,充分分離贅皮處皮膚與眼輪匝肌之間的聯(lián)系,徹底去除錯(cuò)位的眼輪匝肌,分離暴露內(nèi)眥韌帶,將內(nèi)眥韌帶褥式縫合于鼻側(cè)腱膜,使淚阜顯露適中,然后皮瓣轉(zhuǎn)位縫合。 結(jié)果：經(jīng)過對(duì)125例患者電話隨訪4-6個(gè)月顯示：125例患者淚阜顯露適中,內(nèi)眥間距變窄,重瞼線與內(nèi)眥切口處銜接自然。內(nèi)眥部早期有輕微瘢痕增生,于5-6月時(shí)變成淡白色,效果明顯,患者感到滿意。 結(jié)論：切開重瞼術(shù)與內(nèi)眥贅皮矯正術(shù)同時(shí)進(jìn)行,一次設(shè)計(jì)使得內(nèi)眥部二者銜接自然,重瞼弧度圓潤(rùn)；去除錯(cuò)位的眼輪匝肌,從根本上解除了內(nèi)眥贅皮的成因；內(nèi)眥韌帶深層固定更有效降低了贅皮的復(fù)發(fā)率,減小了切口的張力,減輕瘢痕的形成。適用于所有類型內(nèi)眥贅皮。
[Abstract]:Objective: the incidence of epicanthus, also called Mongolian fold, is about 50% in Asian population and more than 70% in single eyelid population. Therefore, epicanthus correction is a common cosmetic surgery. The presence of epicanthus shortens the length of palpebral fissure and does not meet the aesthetic standards of the face. Many surgeons are too afraid to perform the operation for fear of causing visible inner canthus scars. We used Park method, the medial canthus ligament shortening and double eyelid operation performed simultaneously. Through clinical verification, this method is satisfactory for all types of epicanthus correction. Methods: from 2007 to 2009, 125 cases of epicanthus were treated, including 48 cases of upper eyelid type (96 sides), 52 cases of tarsal plate type (104 sides) and 25 cases of lower eyelid type (50 sides). The average age was 30 years old. According to the design line, we cut open the skin, remove the proper amount of skin, fully separate the connection between the skin and the orbicularis oculi muscle, completely remove the misaligned orbicularis oculi muscle, separate the exposed medial canthus ligament, and suture the inner canthus ligament in the aponeurosis of the nasal side. Expose the lacrimal caruncle moderately, then the flap is transposed and sutured. Results: 125 patients were followed up by telephone for 4 to 6 months. The results showed that the lacrimal caruncle of 125 cases was moderate, the distance between the inner canthus became narrow, and the double eyelid line connected naturally with the incision of the inner canthus. The medial canthus had slight scar hyperplasia in the early stage and became light white in May and June. The effect was obvious and the patient was satisfied. Conclusion: double eyelid incision and epicanthus correction are performed at the same time, one design makes the inner canthus join naturally, the double eyelid arc is smooth, remove the misplaced orbicularis oculi muscle, and fundamentally remove the cause of the epicanthus epicanthus. The deep fixation of medial canthus ligament can reduce the recurrence rate of epicanthus, reduce the tension of incision and reduce the formation of scar. Suitable for all types of epicanthus.
