斜視性和屈光參差性弱視患者視覺缺損模式的研究

【摘要】:背景與目的 弱視是視覺發(fā)育敏感期異常視覺經(jīng)驗(yàn)所致的以空間視力(中心視力、空間定位能力、對(duì)比敏感度)損害為特征的一組視力不良綜合征。由于基礎(chǔ)理論薄弱、研究手段局限,我國(guó)對(duì)弱視的研究主要集中在臨床療效觀察與分析、流行病學(xué)調(diào)查、科普性文獻(xiàn)等方面,近20年來(lái),隨著科技發(fā)展,研究方法的突破,使弱視心理物理學(xué)研究逐漸成為認(rèn)知神經(jīng)科學(xué)的一個(gè)前沿和熱點(diǎn)問題。不同類型弱視除具有不同的神經(jīng)機(jī)制外,在視功能方面也存在差異,本文主要比較斜視性和屈光參差性弱視患者的各視功能特征,以分析不同類型弱視存在的視功能缺損模式,從心理物理學(xué)角度來(lái)探討兩種類型弱視機(jī)制的異同。 方法 本文應(yīng)用心理物理學(xué)原理,設(shè)計(jì)了光柵視敏度、邊緣對(duì)比敏感度、雙眼融合、對(duì)比敏感度函數(shù)等視功能檢測(cè)方法,對(duì)125例8-40歲的弱視患者,進(jìn)行視功能檢測(cè),然后運(yùn)用Origin8.0軟件對(duì)實(shí)驗(yàn)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,采用了Pearson、Spearman、t檢驗(yàn)、線性回歸等分析方法,分析光柵視敏度,邊緣對(duì)比敏感度,雙眼融合功能與視覺敏銳度的相關(guān)性,并對(duì)斜視性弱視和屈光參差性弱視患者CSF曲線進(jìn)行比較。 結(jié)果 1總體樣本的光柵視敏度與視覺敏銳度相關(guān)系數(shù)為0.81,呈高度相關(guān)性,弱視眼相對(duì)于正常眼其光柵視力缺損程度輕于optotype視力,兩者最佳擬合直線斜率為0.55,斜視性弱視樣本的散點(diǎn)圖分布約低于總體趨勢(shì)的10%; 2總體樣本的邊緣對(duì)比敏感度與視覺敏銳度的相關(guān)系數(shù)為0.33,呈低度相關(guān)性。當(dāng)optotype視力明顯受損時(shí),邊緣對(duì)比敏感度僅輕度受到損害(斜率=0.26)。屈光參差性樣本的對(duì)比敏感度閾值的分布約高于(或差于)總體趨勢(shì)的12%,斜視及混合性弱視樣本的對(duì)比敏感度閾值分布約低于(或好于)總體趨勢(shì)的20%; 3兩種類型弱視CSF曲線均較相對(duì)健眼的CSF低平,峰值向低空間頻率區(qū)移位,在中高空間頻率區(qū)有明顯受損,無(wú)顯著性差異,但在低頻區(qū),兩者有統(tǒng)計(jì)學(xué)意義; 4在相同光柵視力損害情況下,有雙眼視功能組與無(wú)雙眼視功能組的optotype視力兩兩比較,有顯著性差異(P0.01),即有雙眼視功能的optotype視力要優(yōu)于無(wú)雙眼視功能的optotype視力。 結(jié)論 兩種不同類型的弱視存在不同的視覺缺損模式。
[Abstract]:Background and purposeAmblyopia is a group of dysacuity syndrome characterized by the impairment of visual acuity (central vision, spatial location ability, contrast sensitivity) due to the abnormal visual experience in the sensitive period of visual development.Because of the weak basic theory and limited research methods, the research on amblyopia in our country is mainly focused on clinical curative effect observation and analysis, epidemiological investigation, popular science literature, etc. In the past 20 years, with the development of science and technology, the research methods have made a breakthrough.The study of amblyopia psychophysics has gradually become a frontier and hot issue in cognitive neuroscience.Different types of amblyopia not only have different neural mechanisms, but also have different visual functions. In this paper, we compared the visual function characteristics of strabismus and anisometropic amblyopia, in order to analyze the pattern of visual function defect in different types of amblyopia.This paper discusses the similarities and differences between the two types of amblyopia from the point of view of psychophysics.MethodBased on the principle of psychophysics, the visual function of 125 cases of amblyopia aged 8-40 years was measured by using the methods of grating visual acuity, edge contrast sensitivity, binocular fusion and contrast sensitivity function.Then Origin8.0 software was used to analyze the experimental data. Pearsonian Spearmanton test and linear regression were used to analyze the correlation between grating visual acuity, edge contrast sensitivity, binocular fusion function and visual acuity.The CSF curves of strabismus amblyopia and anisometropia amblyopia were compared.Result(1) the correlation coefficient between grating visual acuity and visual acuity of the total sample was 0.81, which was highly correlated. The degree of grating visual acuity defect in amblyopic eyes was lighter than that of optotype visual acuity in amblyopia compared with normal eyes.The optimum fitting slope was 0.55, and the scattered plot distribution of strabismus amblyopia sample was about 10% lower than that of the general trend.2 the correlation coefficient between edge-contrast sensitivity and visual acuity was 0.33, which was low correlation.When the visual acuity of optotype was obviously impaired, the edge contrast sensitivity was only slightly impaired (slope = 0.26).The distribution of contrast sensitivity threshold of anisometropia samples was about 12 higher than (or worse) than that of the general trend, and that of strabismus and mixed amblyopia samples was about 20% lower than (or better) than that of the general trend.(3) the CSF curves of the two types of amblyopia were lower than those of the relatively healthy eyes, and the peak value shifted to the low spatial frequency region, which was obviously damaged in the middle and high spatial frequency region, but there was no significant difference in the low frequency region between the two types of amblyopia, but they had statistical significance in the low frequency region.4 under the same condition of grating visual impairment, the optotype visual acuity of binocular visual function group was significantly higher than that of non-binocular visual function group (P0.01), that is, the optotype visual acuity with binocular visual function was better than optotype visual acuity without binocular visual function.ConclusionTwo different types of amblyopia have different visual defect patterns.
