【摘要】： 目的:探討以FFA(眼底血管熒光造影)計(jì)算機(jī)圖像信息為引導(dǎo)進(jìn)行糖尿病視網(wǎng)膜病變個(gè)體化激光光凝術(shù)對(duì)視網(wǎng)膜功能的影響。 方法:采用隨機(jī)對(duì)照方法,選取符合眼底激光治療適應(yīng)癥的中度非增殖期糖尿病視網(wǎng)膜病變(DR)患者60例(110眼),根據(jù)激光治療的方式不同分成兩組,以FFA計(jì)算機(jī)信息為引導(dǎo)個(gè)體化治療組(實(shí)驗(yàn)組)和傳統(tǒng)FFA圖片下行全視網(wǎng)膜光凝治療組(對(duì)照組),每組30例,隨訪(fǎng)24周。主要觀察指標(biāo):術(shù)后視野光敏度變化、黃斑水腫、中心視力、毛細(xì)血管無(wú)灌注區(qū)、微血管瘤及新生血管等。 結(jié)果:兩組術(shù)后視網(wǎng)膜光敏度下降,實(shí)驗(yàn)組下降1.23±0.47 dB,對(duì)照組下降2.88±0.68 dB,下降程度試驗(yàn)組明顯低于對(duì)照組(p0.05);與激光術(shù)前相比實(shí)驗(yàn)組差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05),對(duì)照組差異有統(tǒng)計(jì)學(xué)意義(p0.05)。隨訪(fǎng)期內(nèi)持續(xù)性黃斑水腫8眼其中實(shí)驗(yàn)組3,對(duì)照組5眼;中心視力改變、毛細(xì)血管無(wú)灌注區(qū)、微血管瘤消退、新生血管等兩組差異均無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)。 結(jié)論:視網(wǎng)膜光凝術(shù)對(duì)視野造成一定程度損害;個(gè)體化激光治療方案能有效達(dá)到視網(wǎng)膜光凝治療目的,而視網(wǎng)膜功能損傷明顯小于對(duì)照組;應(yīng)用個(gè)體化激光治療方案無(wú)明顯增加早期糖尿病視網(wǎng)膜病變視力喪失和其他并發(fā)癥的風(fēng)險(xiǎn)。
[Abstract]:Objective: to investigate the effect of individualized laser photocoagulation on retinal function in diabetic retinopathy guided by FFA (fundus fluorescein angiography) image information.Methods: sixty patients with moderate nonproliferative diabetic retinopathy (DRR) were randomly divided into two groups according to the different methods of laser therapy.The whole retinal photocoagulation therapy group (control group, 30 cases) was followed up for 24 weeks with FFA computer information as the guide of individualized treatment group (experimental group) and traditional FFA image as the whole retinal photocoagulation treatment group (control group, 30 cases).Main outcome measures: postoperative visual field changes, macular edema, central visual acuity, capillary no perfusion area, microangioma and neovascularization.Results: after operation, the degree of retinal Guang Min in the two groups decreased 1.23 鹵0.47 dB in the experimental group and 2.88 鹵0.68 dB in the control group, which was significantly lower in the experimental group than in the control group, but there was no significant difference between the experimental group and the control group (P 0.05).There were 3 eyes in the experimental group and 5 eyes in the control group during the follow-up period. There was no significant difference in central visual acuity, capillary perfusion area, microangioma regression and neovascularization between the two groups (P 0.05).Conclusion: retinal photocoagulation has a certain degree of damage to the visual field, individualized laser therapy can effectively achieve the purpose of retinal photocoagulation, and retinal function damage is obviously smaller than that of the control group.Individualized laser therapy did not significantly increase the risk of vision loss and other complications in early diabetic retinopathy.
