【摘要】：目的對主觀性耳鳴患者的聽力損失進(jìn)行臨床分析,進(jìn)一步探討耳鳴的發(fā)病原因,為檢查方法提供臨床依據(jù)。方法對以主觀性耳鳴為主訴的200名患者進(jìn)行耳科專科檢查,聲導(dǎo)抗檢測,常頻純音聽閾測聽和高頻擴(kuò)展聽閾測聽檢查。結(jié)果200例耳鳴患者中,聽力正常者151耳(55%),輕度聾73耳(26%),中度聾26耳(13%),中重度聾15耳(5%),重度聾2耳(1%)；單側(cè)耳鳴123例(61.5%)；雙側(cè)耳鳴77例(38.5%)；單側(cè)耳鳴者中,患側(cè)與對側(cè)閾值比較0.125KHz-8KHz有統(tǒng)計學(xué)意義(P0.05),10kHz及其以上頻率無統(tǒng)計學(xué)意義(P0.05)；常頻聽力正常組與聽力異常組比較各頻率閾值均具有統(tǒng)計學(xué)意義(P0.05),聽力異常組閾值檢出率低于聽力正常組。結(jié)論耳鳴好發(fā)年齡段是31-60歲,隨年齡增加聽力減退的患病率逐漸增加,但耳鳴的患病率并未相應(yīng)的增加；大部分耳鳴患者語言頻率未受累積或影響不大,不影響日常交流；耳鳴可以存在于聽力正常人群中；擴(kuò)展高頻測聽能否為耳鳴患者提供早期聽力損失的依據(jù)需要更多的臨床資料進(jìn)一步研究。
[Abstract]:Objective to analyze the hearing loss of patients with subjective tinnitus and explore the causes of tinnitus.Methods 200 patients with subjective tinnitus were examined with special otology, acoustic impedance test, normal frequency pure tone threshold audiometry and high frequency expanded audiometry.Compared with the contralateral threshold, 0.125KHz-8KHz had no significant difference in the frequency of 10 kHz or more than that in the normal group and the normal frequency group. The threshold of each frequency in the normal hearing group was significantly higher than that in the abnormal hearing group, and the detection rate of the threshold in the abnormal hearing group was lower than that in the normal hearing group.Conclusion the prevalence rate of hearing loss increases gradually with age, but the prevalence of tinnitus does not increase correspondingly, and the language frequency of most tinnitus patients is not accumulated or affected, and does not affect daily communication.Tinnitus may exist in the normal hearing population, and whether the extended high frequency audiometry can provide the basis of early hearing loss for tinnitus patients needs more clinical data.
