【摘要】：目的：通過對益腎活血通竅湯合西藥(西比靈、谷維素)治療腎精虧損型感音神經(jīng)性耳鳴的臨床療效進(jìn)行觀察,研究益腎活血通竅湯的臨床價(jià)值,為中西醫(yī)結(jié)合治療腎精虧損型感音神經(jīng)性耳鳴提供臨床思路。 方法：將腎精虧損型感音神經(jīng)性耳鳴的患者58例,隨機(jī)分為治療組、對照組,其中治療組29例37耳,對照組29例38耳。治療組口服中藥益腎活血通竅湯合西藥(西比靈、谷維素)治療,對照組單純口服西藥(西比靈、谷維素)治療,15天為一療程,兩組均連續(xù)用藥30天,并對患者進(jìn)行臨床隨訪1個(gè)月,按照制定的療效評定標(biāo)準(zhǔn),分別觀察兩組療效。 結(jié)果： 1.兩個(gè)療程結(jié)束后,并對患者進(jìn)行臨床隨訪1個(gè)月,治療組和對照組治療前后耳鳴程度癥狀分級量化積分進(jìn)行組間比較,差異有統(tǒng)計(jì)學(xué)意義；治療組總有效率82.99%,對照組總有效率54.78%,經(jīng)統(tǒng)計(jì)學(xué)觀察兩組患者療效,有統(tǒng)計(jì)學(xué)意義(P0.05)； 2.治療組耳鳴療效與聽力障礙存在及其輕重程度有明顯關(guān)系(P0.05)； 3.治療組耳鳴療效與病程有關(guān),病程3月、病程4月-1年、病程1年總有效率分別為：95.24%、75.0%、50.0%，，有統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論： 1.治療組和對照組治療腎精虧損型感音神經(jīng)性耳鳴均具有一定的療效,治療組療效明顯優(yōu)于對照組。 2.治療組耳鳴療效與聽力障礙存在及其輕重程度有明顯關(guān)系。 3.治療組耳鳴療效與病程有關(guān),且療程越短療效越好。 4.經(jīng)驗(yàn)方益腎活血通竅湯以補(bǔ)腎填精,活血通竅為主要功用,是治療感音神經(jīng)性耳鳴腎精虧損證的有效方劑,與西比靈、谷維素組相比,改善耳鳴及其伴隨癥狀效果更明顯,因而具有一定的優(yōu)勢。 5.腎精虧損型感音神經(jīng)性耳鳴患者與年齡關(guān)系密切,且以老年人發(fā)病最為多見,與性別無明顯關(guān)系。 6.治療組在治療前、后均進(jìn)行血、尿常規(guī),肝、腎功能,常規(guī)心電圖等安全性檢測,均未發(fā)現(xiàn)不良反應(yīng),表明益腎活血通竅湯合西藥(西比靈、谷維素)治療腎精虧損型感音神經(jīng)性耳鳴是中西醫(yī)結(jié)合治療耳鳴安全可靠的方法,值得臨床推廣應(yīng)用。
[Abstract]:Objective: to study the clinical value of Yishen Huoxue Tongqiao decoction (Yishen Huoxue Tongqiao decoction) combined with western medicine (sibeiling, oryzanol) in the treatment of sensorineural tinnitus with deficiency of kidney essence, and to study the clinical value of Yishen Huoxue Tongqiao decoction.To provide clinical thinking for the treatment of sensorineural tinnitus with deficiency of kidney essence by combination of traditional Chinese and western medicine.Methods: 58 cases of sensorineural tinnitus with deficiency of kidney essence were randomly divided into two groups: treatment group (n = 29), control group (n = 37) and control group (n = 29).The treatment group was treated with traditional Chinese medicine Yishen Huoxue Tongqiao decoction and western medicine (sibeiling, oryzanol), while the control group was treated with oral western medicine (sibelium, oryzanol) for 15 days as a course of treatment. The two groups were treated continuously for 30 days.The patients were followed up for 1 month.Results:1.After two courses of treatment, the patients were followed up for 1 month. The scores of tinnitus degree were compared between the treatment group and the control group before and after treatment, the difference was statistically significant.The total effective rate was 82.99 in the treatment group and 54.78 in the control group.2.The curative effect of tinnitus in the treatment group was significantly related to the presence and severity of hearing impairment.3.The curative effect of tinnitus in the treatment group was related to the course of disease. The total effective rates of the course of disease were 3 months, 4 months to 1 year, and the total effective rate of one year was 50.0, respectively.Conclusion:1.Both the treatment group and the control group were effective in the treatment of sensorineural tinnitus with deficiency of kidney essence, and the curative effect of the treatment group was significantly better than that of the control group.2.The curative effect of tinnitus in the treatment group was significantly related to the existence and severity of hearing impairment.3.The curative effect of tinnitus in the treatment group was related to the course of disease, and the shorter the course of treatment, the better the curative effect.4.Experience prescription Yishen Huoxue Tongqiao decoction, which is mainly used to invigorate kidney, activate blood circulation and clear orifices, is an effective prescription for the treatment of deficiency syndrome of sensorineural tinnitus and kidney essence. Compared with sibeiling and oryzol groups, the effect of improving tinnitus and its associated symptoms is more obvious.As a result, there are certain advantages.5.The patients with sensorineural tinnitus of deficiency type of kidney essence were closely related to age, and the elderly were the most common, but had no obvious relationship with sex.6.Before and after treatment, blood, urine routine, liver, kidney function, routine electrocardiogram and other safety tests were performed in the treatment group. No adverse reactions were found, indicating that Yishen Huoxue Tongqiao decoction combined with western medicine (Sibiline),Oryzanol) is a safe and reliable method for the treatment of tinnitus with deficiency of kidney essence.
