【摘要】：目的:評價準分子激光角膜原位磨鑲術(laser in situ keratomileusis,LASIK)治療近視的遠期療效,探討影響療效的相關因素。 方法:選取2002年1月～2002年12月在大連醫(yī)科大學附屬第一醫(yī)院準分子激光治療中心應用德國科醫(yī)人公司的鷹視準分子激光治療系統(tǒng)WaveLight進行LASIK治療的近視眼患者,對其進行隨訪復查,資料完整者37例(71眼),平均年齡25.42±5.97歲。其中男性18人,35眼,占49.30%;女性19人,36眼,占50.70%。術前屈光度-2.25D～-11.75D,平均屈光度-6.06±3.20D。比較手術前后裸眼視力、屈光度的變化,分析影響術后遠期療效的相關因素。 結果:(1)所有術眼術后10年裸眼視力平均值為0.82±0.28,術后10年裸眼視力較術前均顯著提高(P=0.000),術后10年裸眼視力≥1.0和0.5的百分比分別為46.38%和91.30%,視力有效指數(shù)為0.86。(2)所有術眼術后10年屈光度平均值為-0.79±1.42D,術后屈光度較術前均顯著降低(P=0.000),術后10年屈光度在±1.O0D和±O.50D以內的百分比分別為72.50%和50.70%。(3)術后10年平均最佳矯正視力為0.97±0.14D,達到術前最佳矯正視力(P㧐0.05),視力安全指數(shù)為1.02。術后10年最佳矯正視力與術前最佳矯正視力保持不變者占70.42%(50/71);較術前上升1行者占18.31%(13/71);下降1行者占8.45%(6/71);下降2行及以上者占2.82%(2/71)。(4)術后10年主訴眼干、易疲勞、異物感6眼。(5)術后10年有28.17%(20/71)出現(xiàn)屈光回退,屈光回退率與術前屈光度、術前術后角膜厚度差值有相關性。(6)術后10年發(fā)現(xiàn)圓錐角膜1例1眼。 結論:(1)LASIK治療近視術后10年的有效性和安全性好。(2)隨術后時間的推移,有部分患者發(fā)生屈光回退。影響屈光回退的因素為術前屈光度、術前術后角膜厚度差值。(3)LASIK術后10年發(fā)現(xiàn)繼發(fā)圓錐角膜的病例,雖然詳細的術前篩查,術中角膜瓣制作良好和預留足夠厚基質床厚度,以及術后的定期隨訪等是預防繼發(fā)性圓錐角膜的重要措施,但其病因仍需要我們進一步研究。
[Abstract]:Objective: to evaluate the long term effect of laser in situ keratomileusissis LASIKI for myopia, and to explore the related factors affecting the effect of laser in situ keratomileusis (LASK). Methods: myopia patients who were treated with Eagle excimer laser therapy system (WaveLight) from January 2002 to December 2002 in the first affiliated Hospital of Dalian Medical University were treated with LASIK. The data were complete in 37 cases (71 eyes) with an average age of 25.42 鹵5.97 years, including 18 males (35 eyes) and 19 females (36 eyes, 50.70D). The preoperative diopter was -2.25 DV -11.75 D, and the mean diopter was -6.06 鹵3.20 D. the changes of naked visual acuity and diopter before and after operation were compared. To analyze the related factors affecting the long-term outcome after operation. Results the mean uncorrected visual acuity of all eyes was 0.82 鹵0.28 in 10 years after operation.
