【摘要】：目的:探討突發(fā)性耳聾分型治療的必要性以及影響突發(fā)性耳聾治療效果的預(yù)后因素。方法:回顧性分析2011年2月至2013年5月間我科收治的498例(526耳)突發(fā)性耳聾的病例資料。按照患者的聽力曲線將其分為低中頻下降型、中高頻下降型、平坦型和全聾型4種類型。每種類型采用不同治療方案,分析不同類型患者的治療效果,并探討其預(yù)后的影響因素。結(jié)果:共收集病例498例(526耳),男女比例為0.95∶1;年齡13~86歲,平均年齡(45.39±15.78)歲。根據(jù)聽力曲線分型:低中頻下降型91耳(17.30%),中高頻下降型74耳(14.07%),平坦型191耳(36.31%),全聾型170耳(32.32%)。各種類型的總有效率:低中頻型總有效率最高,為70.33%;中高頻型總有效率最低,為35.14%;平坦型50.26%,全聾型51.76%,差異有統(tǒng)計(jì)學(xué)意義(χ2=20.918,P=0.000)。多因素logistic回歸分析結(jié)果顯示:年齡(OR=0.977,95%CI=0.965~0.989,P=0.000)、突發(fā)性耳聾分型(OR=0.977,95%CI=0.180~0.748,0.372~1.199,0.300~0.963,P=0.035)、患耳(OR=0.977,95%CI=0.450~0.989,0.161~0.628,P=0.002)、發(fā)病至干預(yù)時(shí)間(OR=0.977,95%CI=0.925~0.967,P=0.000)與突發(fā)性耳聾治療效果相關(guān)。結(jié)論:突發(fā)性耳聾患者的預(yù)后與年齡、突發(fā)性耳聾分型、患耳、發(fā)病至干預(yù)時(shí)間、高血壓有關(guān)。不同類型的突發(fā)性聾患者療效差異大,低中頻下降型療效最好,全聾型、平坦型次之,中高頻下降型療效不佳。根據(jù)突發(fā)性耳聾患者的聽力曲線進(jìn)行分型,進(jìn)而選擇不同的治療方案具有重要意義。
[Abstract]:Objective: to investigate the necessity of classified treatment of sudden deafness and the prognostic factors influencing the therapeutic effect of sudden deafness. Methods: retrospective analysis of 498 cases of sudden deafness from February 2011 to May 2013 in our department was carried out. According to the hearing curve of the patients, they were divided into low intermediate frequency descent type, There were four types of middle and high frequency descending type, flat type and full deafness type. Each type was treated with different treatment regimens, and the therapeutic effects of different types of patients were analyzed. Results: a total of 498 cases with 526 ears were collected. The ratio of male to female was 0.95: 1. The average age was 45.39 鹵15.78 years. According to the classification of hearing curve, 91 ears with low intermediate frequency descending type had 17.30 ears, 74 ears with medium and high frequency descending type with 14.07 ears, 36.31 ears with flat type 191 ears with 36.31 ears, 170 ears with full deafness type with 32.322.The total effective rate of all kinds of types was the highest in low intermediate frequency type. Prognosis and age of patients with sudden deafness, Sudden deafness type, affected ears, onset to intervention time, hypertension. Different types of sudden deafness patients have great difference in curative effect, low intermediate frequency descending effect is the best, full deafness type, flat type is the second. According to the hearing curve of patients with sudden deafness, it is of great significance to choose different treatment plans.
