【摘要】：目的探討環(huán)孢霉素A聯(lián)合大劑量糖皮質(zhì)激素治療重度活動(dòng)性甲狀腺相關(guān)眼病(thyroid-associated ophthalmopathy, TAO)的臨床療效與安全性。 方法對(duì)2010-2012年在廣西醫(yī)科大學(xué)第一附屬醫(yī)院眼科確診為重度活動(dòng)性TAO,經(jīng)單純糖皮質(zhì)激素治療效果不佳的患者16例,采用環(huán)孢霉素A聯(lián)合大劑量糖皮質(zhì)激素治療。觀察治療前后患者的視力、臨床活動(dòng)性積分、眼壓、視覺(jué)電生理、色覺(jué)、眼部癥狀改善情況及并發(fā)癥發(fā)生情況。 結(jié)果經(jīng)治療后,患者臨床活動(dòng)性積分明顯下降,治療前后比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)；視力明顯提高,治療前后比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)；大部分患者的眼壓、視覺(jué)電生理、色覺(jué)、結(jié)膜充血、眼瞼腫脹明顯好轉(zhuǎn)或消失,且全身并發(fā)癥少。 結(jié)論對(duì)單純大劑量糖皮質(zhì)激素治療效果不佳的重度活動(dòng)性TAO患者,加用環(huán)孢霉素A治療,可有效的控制患者免疫性炎癥,降低患者的臨床活動(dòng)性積分、提高視力且并發(fā)癥少,是安全有效的治療方法。
[Abstract]:Objective to investigate the clinical efficacy and safety of cyclosporine A (cyclosporine A) combined with high dose glucocorticoid in the treatment of severe active thyroid associated ophthalmopathy (TAOO). Methods Sixteen patients diagnosed as severe active TAO in the first affiliated Hospital of Guangxi Medical University from 2010 to 2012 were treated with glucocorticoid alone. The visual acuity, clinical activity score, intraocular pressure, visual electrophysiology, color perception, ocular symptom improvement and complications were observed before and after treatment with cyclosporine A and high dose glucocorticoid. Results after treatment, the clinical activity score of the patients decreased significantly, the difference was statistically significant before and after treatment, the visual acuity was significantly improved, the difference was statistically significant before and after treatment, the majority of patients had intraocular pressure, visual electrophysiology, and visual electrophysiology. Color vision, conjunctival hyperemia, eyelid swelling significantly improved or disappeared, and less systemic complications. Conclusion for the patients with severe active TAO who are not well treated by high dose glucocorticoid alone, the addition of cyclosporine A can effectively control the immune inflammation of the patients, reduce the clinical activity score, improve the visual acuity and have fewer complications. Is a safe and effective treatment.
