【摘要】：目的介紹完壁式乳突切開鼓室成形術(shù)治療活動期慢性化膿性中耳炎的臨床效果。方法回顧分析我院采用完壁式乳突切開鼓室成形術(shù)治療的57例(耳)活動期慢性化膿性中耳炎的臨床資料。結(jié)果48耳術(shù)后鼓膜一期愈合,5耳移植筋膜邊緣殘留裂隙,2-3周后延期愈合,4耳術(shù)后感染鼓膜穿孔未愈合;隨訪10個月至34個月,無復發(fā)病例,57耳的0.5、1.0、2.0、4.0k Hz氣導聽閾平均值由術(shù)前的49.7±9.4d BHL恢復到30.3±8.0 d BHL,氣骨導差平均縮小15 d BHL,其中48耳聽力有改善。結(jié)論采用完壁式乳突切開鼓室成形術(shù)治療活動期慢性化膿性中耳炎,亦可有效清除中耳乳突病灶,重建中耳傳導結(jié)構(gòu),且能保留耳部正常形態(tài),提升患者聽力及生活質(zhì)量。
[Abstract]:Objective to introduce the clinical effect of tympanoplasty with wall mastoidectomy in the treatment of chronic suppurative otitis media in active stage. Methods 57 cases of active chronic otitis media treated by wall mastoidotomy tympanoplasty in our hospital were analyzed retrospectively. Results the primary healing of tympanic membrane in 48 ears and the residual fissure at the edge of the transplanted fascia in 5 ears were delayed for 2-3 weeks. The infection of tympanic membrane perforation was not healed in 4 ears after operation. They were followed up for 10 to 34 months. In 57 ears with no recurrence, the mean hearing threshold of 0.5-1.0 ~ 2.0 ~ 4.0 kHz was recovered from 49.7 鹵9.4 d BHL to 30.3 鹵8.0 d BHL.The air-bone conductance difference was reduced by 15 d on average, and hearing was improved in 48 ears. Conclusion the tympanoplasty with wall mastostomy can be used to treat the patients. Chronic suppurative otitis media, It can also effectively clear the middle ear mastoid lesions, reconstruct the conduction structure of the middle ear, preserve the normal shape of the ear, and improve the hearing and quality of life of the patients.
