【摘要】：目的:評價帕瑞昔布鈉和噴他佐辛超前鎮(zhèn)痛應(yīng)用于鼻內(nèi)鏡手術(shù)術(shù)中及術(shù)后的鎮(zhèn)痛效應(yīng),為臨床合理應(yīng)用超前鎮(zhèn)痛提供參考。方法:以120例局部麻醉鼻內(nèi)鏡下行鼻中隔矯正加鼻甲射頻消融術(shù)的患者為研究對象,隨機分為帕瑞昔布鈉組(A組)、噴他佐辛組(B組)及對照組(C組),每組40例;分別于術(shù)前30min單次肌內(nèi)注射帕瑞昔布鈉40mg、噴他佐辛30mg及相同容積生理鹽水。采用視覺模擬評分(VAS)評估3組患者術(shù)中和術(shù)后3、24、48h的疼痛情況,同時記錄術(shù)后追加鎮(zhèn)痛藥使用情況及藥物相關(guān)不良反應(yīng)的發(fā)生情況。結(jié)果:與C組相比,A組及B組在術(shù)中和術(shù)后3、24、及48h的VAS均顯著降低(P0.05)。與C組相比,A組與B組術(shù)后追加鎮(zhèn)痛藥使用率減少(P0.05)。結(jié)論:局部麻醉鼻內(nèi)鏡下鼻中隔矯正加鼻甲射頻消融術(shù)前給予帕瑞昔布鈉和噴他佐辛均可收到良好的超前鎮(zhèn)痛效果,安全性較高,值得在臨床工作中推廣應(yīng)用。
[Abstract]:Objective: To evaluate the analgesic effect of parecoxib sodium and pentazocine preemptive analgesia in nasal endoscopic surgery and after the surgery, to provide reference for clinical rational use of preemptive analgesia. Methods: 120 patients with local anesthesia plus endoscopic nasal turbinate radiofrequency ablation as the research object, randomly divided into parecoxib celecoxib sodium group (A group), pentazocine group (B group) and control group (C group), 40 cases in each group respectively; preoperative 30min single intramuscular injection of parecoxib sodium 40mg, pentazocine 30mg and the same volume of saline. Using visual analogue score (VAS) pain 3,24,48h and postoperative assessment of 3 groups of patients were recorded at the same time, occurrence of post-operative analgesic drug use and drug related adverse reactions. Results: compared with C group, A group and B group in the intraoperative and postoperative 3,24, 48h and VAS were significantly decreased (P0.05). Compared with the C group. A group and B group after operation Supplementary analgesia was decreased (P0.05). Conclusion: local anesthesia in endoscopic nasal septum and inferior turbinate radiofrequency ablation before parecoxib sodium and pentazocine can receive better analgesia effect, high safety, and is worthy of popularization and application in clinical work.
