【摘要】：目的:探討多導(dǎo)睡眠監(jiān)測(cè)(PSG)和頭顱側(cè)位片(LCR)在兒童睡眠呼吸障礙(SDB)診治中的應(yīng)用。方法:選擇SDB患兒157例,其中單純性打鼾(PS)組115例,阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)組42例。分別行雙側(cè)扁桃體切除和腺樣體切除,術(shù)前、術(shù)后行PSG和LCR檢查,對(duì)各項(xiàng)測(cè)量指標(biāo)進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:PS組和OSAHS組腺樣體厚度的差異有統(tǒng)計(jì)學(xué)意義(P0.01);鼻咽氣道斜徑術(shù)前差異有統(tǒng)計(jì)學(xué)意義(P0.05)。PS組和OSAHS組鼻咽氣道斜徑和口咽氣道前后徑術(shù)前術(shù)后的差異均有統(tǒng)計(jì)學(xué)意義(均P0.01)。OSAHS組AHI及LSaO2術(shù)前術(shù)后差異均有統(tǒng)計(jì)學(xué)意義(均P0.01)。PS組總有效率為97.4%(112/115),OSAHS組總有效率為97.6%(41/42)。結(jié)論:PSG和LCR在兒童SDB診治中是重要的參考指標(biāo)。雙側(cè)扁桃體和腺樣體切除是治療兒童SDB的主要手段。
[Abstract]:Objective: to investigate the application of polysomnography (PSG) and lateral cephalic radiographs (LCRs) in the diagnosis and treatment of sleep apnea disorder (SDBs) in children. 42 patients with obstructive sleep apnea hypopnea syndrome underwent bilateral tonsillectomy and adenoidectomy. PSG and LCR were performed before and after operation. Results there were significant differences in adenoid thickness between the OSAHS group and the PS group (P 0.01), and there were significant differences in the oblique diameter of nasopharynx airway before operation (P 0.05). PS group and OSAHS group (P 0.05) and OSAHS group (P < 0 05) had significant difference in nasopharyngeal airway oblique diameter and anterior oropharynx airway diameter. There were statistically significant differences in posterior diameter before and after operation (P 0.01g 路OSAHS group) and LSaO2 group (P 0.01g 路PS group). The total effective rate was 97.4 / 115.The total effective rate of OSAHS group was 97.60.46% 42.Conclusion the ratio of AHI and LCR is important in the diagnosis and treatment of SDB in children. Bilateral tonsillectomy and adenoidectomy are the main methods for the treatment of SDB in children.
