【摘要】：目的評價血清IgG4濃度對IgG4相關(guān)性疾病的鑒別診斷價值,并探討IgG4相關(guān)性疾病的眼部表現(xiàn)。方法收集IgG4相關(guān)性疾病患者29例,血清IgG4濃度升高的其他疾病患者161例(其中自身免疫性肝病47例,急性胰腺炎15例,原發(fā)性腎小球病及腎功能不全61例,炎癥性腸病38例),正常對照190例;以免疫散射比濁法測定血清IgG4和IgG濃度,比較各疾病組患者的檢測結(jié)果。以所有患者的血清IgG4濃度繪制受試者工作特征(ROC)曲線,評價血清IgG4濃度的診斷價值,并確定其診斷IgG4相關(guān)性疾病的最佳臨界值。同時對IgG4相關(guān)性疾病患者進(jìn)行眼科檢查,評估其眼部表現(xiàn)。結(jié)果 IgG4相關(guān)性疾病組血清IgG4濃度與原發(fā)性腎小球病及腎功能不全組、炎癥性腸病組、自身免疫性肝病組、急性胰腺炎組比較,差異有統(tǒng)計學(xué)意義(P0.01)。經(jīng)ROC曲線分析,當(dāng)IgG4為2.37 g/L時,Youden指數(shù)最大,該診斷界值的敏感度為93.1%,特異度為84.3%,曲線下面積為0.939。29例IgG4相關(guān)性疾病患者中,2例有眼眶占位性病變,其余患者眼部表現(xiàn)無特異性。結(jié)論血清IgG4濃度對于診斷IgG4相關(guān)性疾病具有較高的敏感度和特異度,但僅能作為診斷IgG4相關(guān)性疾病的依據(jù)之一,提示IgG4相關(guān)性疾病的可能;該病眼部表現(xiàn)無特異性。
[Abstract]:Objective to evaluate the value of serum IgG4 concentration in differential diagnosis of IgG4 associated diseases and to explore the ocular manifestations of IgG4 associated diseases. Methods 29 patients with IgG4 associated diseases were collected. There were 161 patients with other diseases with elevated serum IgG4 concentration (including 47 cases of autoimmune liver disease, 15 cases of acute pancreatitis, 61 cases of primary glomerulonephritis and renal insufficiency, 38 cases of inflammatory bowel disease and 190 cases of normal control). The serum IgG4 and IgG concentrations were measured by immune turbidimetric method, and the results of each disease group were compared. The serum IgG4 concentration of all patients was plotted to evaluate the diagnostic value of serum IgG4 concentration. The best critical value for the diagnosis of IgG4 associated diseases was determined. Ophthalmologic examination was performed to evaluate the ocular manifestations of patients with IgG4 associated diseases. Results the serum IgG4 concentration in IgG4 related diseases was associated with primary glomerulopathy and renal insufficiency. The difference was statistically significant in inflammatory bowel disease group, autoimmune liver disease group and acute pancreatitis group. According to ROC curve analysis, when IgG4 was 2.37 g / L, the Youden index was the largest. The sensitivity of the diagnostic threshold was 93.1 and the specificity was 84.3. The area under the curve was 0.939.29 patients with IgG4 related diseases. Conclusion Serum IgG4 concentration has high sensitivity and specificity in the diagnosis of IgG4 related diseases, but it can only be used as a basis for the diagnosis of IgG4 related diseases, indicating the possibility of IgG4 related diseases. The ocular manifestations of the disease are not specific.
