【摘要】:目的:探討血管內(nèi)皮生長因子(VEGF)、缺氧誘導(dǎo)因子-1α(HIF-1α)在阻塞性睡眠呼吸暫停/低通氣綜合征(OSAHS)患者及其心血管并發(fā)癥發(fā)生中的意義。 方法:對48例OSAHS合并心血管疾病患者、52例單純OSAHS患者和100名體檢正常者進行多導(dǎo)睡眠監(jiān)測,采用酶聯(lián)免疫吸附試驗法對三組對象外周血中的VEGF、HIF-1α水平進行檢測,外周血VEGF mRNA表達水平的測定采用逆轉(zhuǎn)錄-聚合酶鏈反應(yīng)及計算機條帶分析,并進行相關(guān)性分析,運用聚合酶鏈反應(yīng)-限制性片段長度多態(tài)性(PCR-RFLP)技術(shù)檢測三組對象VEGF基因+813 C/T位點基因型。 結(jié)果:(1)三個組別間血漿HIF-1α濃度、VEGF含量、外周血單個核細胞VEGF mRNA的表達比較差異有統(tǒng)計學(xué)意義(P 0.01),兩兩比較顯示單純OSAHS組高于正常對照組,且差異有統(tǒng)計學(xué)意義(P 0.01),OSAHS合并心血管病組高于單純OSAHS組,差異有統(tǒng)計學(xué)意義(P 0.01)。(2)①OASHS患者血漿HIF-1α、VEGF水平以及VEGF mRNA的表達水平與AHI、T-SaO290%、晨起收縮壓、晨起舒張壓呈不同程度正相關(guān),與minSaO2呈負(fù)相關(guān)。②HIF-1α與血漿VEGF濃度、VEGF mRNA表達呈正相關(guān)。(3)與OSAHS患者相比,正常對照組的CT和TT基因型較多,差異有統(tǒng)計學(xué)意義(P 0.01);正常對照組的T等位基因較OSAHS組明顯增多(P 0.01)。OSAHS組以及正常對照組CC基因型患者血漿VEGF水平均較CT+TT基因型患者相比均有升高,差異有統(tǒng)計學(xué)意義(P 0.05)。 結(jié)論:HIF-1α在OSAHS患者外周血中的水平明顯升高,OSAHS合并心血管疾病組的升高更加明顯,主要通過調(diào)節(jié)VEGF的水平,參與OSAHS的發(fā)病,并可能是OSAHS心血管并發(fā)癥的重要發(fā)病機制之一;VEGF基因+813位點出現(xiàn)T等位基因可能降低OSAHS的易感性。
[Abstract]:Objective: To investigate the significance of vascular endothelial growth factor (VEGF) and hypoxia inducible factor -1 alpha (HIF-1 a) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and cardiovascular complications.
Methods: 48 cases of OSAHS patients with cardiovascular disease, 52 cases of OSAHS patients and 100 healthy subjects underwent polysomnography, using enzyme-linked immunosorbent assay method in three groups of VEGF in peripheral blood, detected levels of HIF-1 alpha, determined by polymerase chain reaction and computer reverse transcriptase band analysis the level of expression of VEGF in peripheral blood of mRNA, and the correlation analysis was performed, using polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) technique for the detection of three groups of VEGF gene +813 C/T genotype.
Results: (1) between the three groups of plasma HIF-1 concentration, VEGF content, significant expression of VEGF in peripheral blood mononuclear cells mRNA difference (P 0.01), the 22 comparison shows that pure OSAHS group is higher than the normal control group, and the difference was statistically significant (P 0.01), OSAHS group was higher than that of cardiovascular disease in the OSAHS group, the difference was statistically significant (P 0.01). (2) in the plasma of patients with OASHS HIF-1 alpha, VEGF level and VEGF expression level of mRNA and AHI, T-SaO290%, morning systolic, early diastolic blood pressure was positively related to different degrees, and negatively correlated with minSaO2. The HIF-1 alpha and VEGF concentration in plasma VEGF, mRNA expression was positively correlated. (3) compared with OSAHS patients, CT and TT genotypes were the normal control group, the difference was statistically significant (P 0.01); normal control group T allele significantly increased than group OSAHS (P 0.01).OSAHS group and normal control group in plasma of patients with CC genotype VEGF Compared with the CT+TT genotype, the levels of the patients were all higher than those of the CT+TT genotypes, and the difference was statistically significant (P 0.05).
