【摘要】： 目的：通過觀察VEGF及IL-8在以嗜酸性細(xì)胞為主及以中性粒細(xì)胞為主的鼻息肉中的表達(dá),以分析它們?cè)诒窍⑷獍l(fā)病機(jī)理中的作用。 方法：于2007年6月-2009年12月對(duì)在我院行鼻內(nèi)鏡手術(shù)的240余例鼻息肉患者的標(biāo)本進(jìn)行嚴(yán)格篩選,均為臨床資料完整的石蠟包埋組織。標(biāo)本取材前2周均未應(yīng)用糖皮質(zhì)激素和抗組胺類藥物。選出以嗜酸性細(xì)胞浸潤(rùn)為主及以中性粒細(xì)胞浸潤(rùn)為主的鼻息肉標(biāo)本各32例,其中以嗜酸性細(xì)胞浸潤(rùn)為主的男21例,女10例,年齡14-72歲,平均50.5歲；以中性粒細(xì)胞浸潤(rùn)為主的男19例,女12例,年齡27-69歲,平均48.3歲；對(duì)照組選自單純鼻中隔偏曲患者的正常中鼻甲粘膜為30例,不伴有過敏性鼻炎、慢性鼻炎、慢性化膿性鼻竇炎。 以嗜酸性細(xì)胞為主的鼻息肉32例,以中性粒細(xì)胞為主的鼻息肉32例及正常中鼻甲粘膜30例。將正常中鼻甲粘膜和鼻息肉用10%福爾馬林固定,石蠟包埋,切片。用免疫組織化學(xué)方法檢測(cè)血管內(nèi)皮生長(zhǎng)因子及白介素8在正常中鼻甲粘膜和嗜酸性粒細(xì)胞及中性粒細(xì)胞鼻息肉中的表達(dá)。 結(jié)果：VEGF及IL-8在正常鼻粘膜均呈弱陽(yáng)性,即少見棕黃色顆粒,在嗜酸性粒細(xì)胞及中性粒細(xì)胞鼻息肉中呈陽(yáng)性或強(qiáng)陽(yáng)性表達(dá)。VEGF在以嗜酸性細(xì)胞為主的鼻息肉中呈強(qiáng)陽(yáng)性表達(dá),在以中性粒細(xì)胞為主的鼻息肉中呈陽(yáng)性表達(dá)；IL-8在以中性粒細(xì)胞為主的鼻息肉中呈強(qiáng)陽(yáng)性表達(dá),在以嗜酸性細(xì)胞為主的鼻息肉中呈陽(yáng)性表達(dá)。VEGF和IL-8在鼻息肉中的表達(dá)呈正相關(guān)。 結(jié)論：VEGF在以嗜酸性細(xì)胞為主的鼻息肉中為高表達(dá),IL-8在以中性粒細(xì)胞為主的鼻息肉中高表達(dá),VEGF和IL-8在鼻息肉中的表達(dá)呈正相關(guān)。說明兩種病理類型鼻息肉發(fā)病機(jī)理不同的,VEGF及IL-8在鼻息肉的發(fā)病機(jī)制中起重要作用。
[Abstract]:Aim: to investigate the expression of VEGF and IL-8 in nasal polyps with eosinophils and neutrophils, and to analyze their role in the pathogenesis of nasal polyps. Methods: from June 2007 to December 2009, the specimens of more than 240 cases of nasal polyps underwent endoscopic sinus surgery in our hospital were strictly screened. No glucocorticoids and antihistamines were used 2 weeks before the specimen was collected. The nasal polyps with eosinophil infiltration and neutrophil infiltration were selected respectively. 32 cases. There were 21 males and 10 females with eosinophil infiltration, aged 14-72 years (mean 50.5 years). There were 19 males and 12 females with neutrophil infiltration, aged 27-69 years (mean 48.3 years). In the control group, 30 cases of normal middle turbinate mucosa were selected from patients with simple deviation of nasal septum, without allergic rhinitis, chronic rhinitis and chronic suppurative sinusitis. 32 cases of nasal polyps with eosinophils, 32 cases with neutrophil polyps and 30 cases with normal middle turbinate mucosa were fixed with 10% formalin. The expression of vascular endothelial growth factor (VEGF) and interleukin-8 (IL-8) in normal middle turbinate mucosa, eosinophil and neutrophil nasal polyps were detected by immunohistochemistry. Results both IL-8 and IL-8 were weakly positive in normal nasal mucosa. Positive or strongly positive expression of VEGF in eosinophil and neutrophil nasal polyps. VEGF was strongly expressed in nasal polyps with mainly eosinophil. Positive expression was found in nasal polyps dominated by neutrophils. The expression of IL-8 was strongly positive in nasal polyps with neutrophil and positive correlation with IL-8 in nasal polyps with eosinophilic cells. Conclusion in nasal polyps with eosinophils, the expression of IL-8 is higher than that in neutrophilic polyps. The positive correlation between the expression of VEGF and IL-8 in nasal polyps suggests that the two pathological types of nasal polyps play an important role in the pathogenesis of nasal polyps.
